BackgroundCoronary heart disease (CHD) is a psychosomatic disease which threatens the health of human seriously. In addition to physical symptoms,patients with coronary heart disease have higher incidence of psychic disorders. At the same time, Intervention such as percutaneous coronary angioplasty and stent implantation has become an important treatment of coronary heart disease. However, patients did not fully understand the operation process or worry about the disability to work and family economic burdens, so many patients are anxious, depressed and have other negative emotions. Therefore, we should identify whether there are affective disorders in patients hospitalized with coronary heart disease and its influencing factors, and take psychological intervention measures before surgical intervention. This can reduce anxiety, depression and other negative emotions, relieve physical symptoms, ensure the success of interventional operation, increase intervention efficacy, and improve patient quality of life.Objective(1)To investigate pre-operative and post-operative psychological condition of patients and its influencing factors;(2)Take psychotherapy and clinical conventional measures to improve the psychological state before receiving psychological intervention, and analyze the effect of psychological intervention and provide the evidence of evidence-based medicine for wide clinical applications.Materials & MethodsPatients receiving percutaneous coronary angioplasty (PTCA) and stent implantation were randomly divided into experimental group and control group. The experimental group included 37 patients (male: 27; female: 10), aged 39 to 70 years, mean age (55.88±11.67) years. The control group included 40 patients (male: 29; female: 11), aged 37 to 72 years, mean age (59.60±10.54) years. Before operation, psychological condition of patients in these two groups were investigated with self-rating depression scale (SDS), self-rating anxiety scale (SAS) and the Symptom Checklist (SCL-90). Besides preoperative routine treatment for all patients, psychological intervention was done for patients in experimental group. The psychological state of patients was evaluated two weeks after surgery using the above SDS, SAS and SCL-90. The data was analyzed with statistical software SPSS13.0.Results(1)There were no significantly statistical differences of age , gender, educational level and duration of disease between these two groups(P> 0.05); and there were no statistically significant difference of heart rate and systolic blood pressure before surgery between these two groups (P> 0.05). All patients had no serious physical diseases, such as severe cerebral vascular disease, etc.(2) Before surgery, SDS, SAS and SCL-90 score of patients in two groups were above the threshold of norm, but there were no significant differences between these two groups (P> 0.05).(3) Age, disease awareness and education level related significantly to mental health condition of patients.(4) Compared with preoperative scores, SDS, SAS and SCL-90 scores of patients in the experimental group were significantly lower two weeks after intervention treatment (P <0.01); SAS and SDS scores of patients in control group were higher than those before treatment, but there was significantly differences only in SAS scores. There were no significant differences of SDS and SCL-90 scores before and after PCI in control group.(5) After intervention treatment, SDS, SAS and SCL-90 total score of patients in the experimental group were lower than control group patients. There were significantly statistical differences of SDS, SAS and SCL-90 total score between two groups (P <0.01).Conclusion(1) Patients with coronary heart disease had different degrees of psychological problems before intervention treatment;(2) Age, disease awareness and education levels can affect people's mental health;(3) Preoperative psychological intervention can help patients to improve the psychological state, quality of life and prognosis.
