| Objective:To investigate the expression of thrombin activatable fibrinolysis inhibitor (TAFI) in patients with polycystic ovary syndrome (PCOS), to analyze the relationship of TAFI to insulin resistance (IR), obesity, blood lipids, Coagulation index, the level of sex hormone,To investigate the changes of coagulation and fibrinolysis system in patients with polycystic ovary syndrome and the role of thrombin activatable fibrinolysis inhibitor (TAFI) in the pathogenesis of polycystic ovary syndrome (PCOS).Methods:38 PCOS patients were selected as PCOS group , insulin-resistant group is 22 cases and non-insulin-resistant group is 16 cases, normal control group is 22 cases .1. the antigen level of thrombin activatable fibrinolysis inhibitor (TAFI) in plasma were measured in Enzyme-linked immunosorbent assay (ELISE) ;2. serum follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), progesterone (P), testosterone (T), prolactin (PRL) and serum fasting insulin (FINS ) levels were measured by Chemiluminescence assay, fasting Plasma glucose (FPG) level was measured by glucose oxidase method;3. Total cholesterol (TC), triglyceride (TG) were measured by automatic biochemical analyzer ;thrombin time (TT), prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen (FBG), antithrombinⅢ(AT-Ⅲ) were measured by Automated blood coagulation analyzer in the plasma;4. insulin resistance index was Calculated by Homeostasis model assessment ;5. the relationship of TAFI to insulin resistance (IR), obesity, blood lipids, Coagulation index, the level of sex hormone was analyzed by correlation and regression .Results:1. the plasma TAFI level was 233.3±87.1 ug / l in Insulin resistance group (IR group) ,the plasma TAFI level was 158.6±79.4 ug / l in non-insulin-resistant group (non-IR group) , the plasma TAFI level was 36.5±34.5 ug / l in the control group, Difference among the three groups was significant (p <0.05);2.The LH in IR group, non-IR group and control group were respectively: 13.61±6.90 mIU / ml, 15.82±7.01 mIU / ml, 4.83±2.51 mIU / ml, the T in IR group ,non-IR group and the control group were respectively: 1.01±0.30 ng / ml, 0.64±0.11 ng / ml, 0.47±0.21 ng / ml, LH and T were significantly higher in IR group and non-IR group compared with control group ,the difference was significant (p <0.05), The HOMA-IR in IR group, non-IR group and the control group were respectively: 3.98±2.85,1.30±0.43,1.34±0.76, The BMI in IR group ,non-IR group and the control group were respectively: 25.19±6.76,20.29±2.30, 21.49±3.03,The WHR in IR group ,non-IR group and control group were respectively: 0.85±0.03,0.77±0.04, 0.75±0.03, HOMA-IR, BMI, WHR were significantly higher in IR group compared with non-IR group and control group, The difference was significant (p <0.05),among the three groups serum lipids was no significant difference;3. The coagulation index TT in IR group, non-IR group and the control group were respectively: 16.84±1.21s, 17.09±0.94s ,18.13±2.01s, the AT-III (%) in IR group, non-IR group and the control group were respectively: 107.12±11.31,102.44±9.12, 113.34±15.51,the TT, AT-III was significantly lower in IR group and non-IR group compared with the control group(p<0.05), other Coagulation indexes were no significant differences among the three groups;4. TAFI was positively correlated with HOMA-IR (r = 0.473, P = 0.005),BMI (r = 0.451, p = 0.001),WHR (r = 0.334, p = 0.004),LH (r = 0.527, p = 0.001), FSH (r = 0.339, p = 0.008), T (r = 0.507, p = 0.001),TAFI were negatively correlated with age (r = -0.386, p = 0.002),TT (r =- 0.365, P = 0.004),TAFI was not correlated with other indicators.Conclusions:1. TAFI antigen levels was increased in patients with PCOS, coagulation indexes were altered in patients with PCOS, which may be related with PCOS long-term complications such as cardiovascular disease, diabetes and other;2. TAFI expression is closely related to insulin resistance, may be involved in the development of insulin resistance;3. The expression of TAFI was associated with obesity;4. The expression of TAFI was associated with the changes of Coagulation and fibrinolysis system. |
