ObjectiveThe aim of this study was to investigate the associations of the insulin resistance ,adiponectin levels and prehypertension. A total of 160 cases were enrolled in this study.MethodsAll the cases were divided into 3 groups according to blood pressure: normal blood pressure group(43 cases), prehypertensive group (79 cases) and stage 1 hypertensive group (38 cases). All cases of coronary heart disease, diabetes, hyperlipidemia and inflammatory diseases are ruled out. Their lipids, glucose and other biochemical markers were detected after fasting for 12 hours. Then we detect the ADP level in blood serum in 3 groups by enzyme linked immunosorbent assay(ELLISA) respectively. The insulin level in the same groups was detected by radioimmunoassay (RIA) ,and HOMA-IR was used to calculate insulin resistant index. All data was entered into an Excel database, and then the data would be analyzed by SPSS13.0--a statistical analysis software.Results(1) General information was compared among each group. Sex ratio, smoking rates, drinking rates and triglyceride levels were not significantly different among the groups(P>0.05). BMI (body mass index) in prehypertension group (25.29±0.27kg/m~2)was higher than normal blood pressure group(22.73±0.37kg/m~2). The difference was statistically significant(P<0.01). BMI (body mass index), fasting blood glucose in stage 1 hypertensive group (26.74±0.54 kg/m~2,6.32±0.27mmol/L)were higher than in normal blood pressure group(22.73±0.37 kg/m~2,5.24±0.11mmol/L) and prehypertensive group(25.29±0.27 kg/m~2,5.56±0.07mmol/L). Total cholesterol in stage 1 hypertensive group (4.93±0.12mmol/L)was higher than in normal blood pressure group(4.56±0.06 mmol/L).(2) HOMA-IR and adiponectin levels were compared in all groups. Serum adiponectin levels in prehypertension group(18.59±1.32mg/L) was significantly lower than in normal blood pressure group(22.89±1.25mg/L). Fasting insulin, HOMA-IR in prehypertension group(7.18±0.22mIU/L,1.78±0.06) were significantly higher than in normal blood pressure group(5.98±0.25 mIU/L,1.39±0.06). In stage 1 hypertensive group, adiponectin levels was significantly lower than in normal blood pressure group. Fasting insulin, insulin resistance index in stage 1 hypertensive group were significantly higher than other groups.(3) The Multiple Logistic Regression Analysis showed that when a person who is in normal blood pressure stage progressed to prehypertension, BMI and insulin resistance index are the risk factors. On the other hand, ADP is a protective factor in the same process.HOMA-IR is a risk factor for the process of prehypertension progressing to stage 1 hypertension.Conclusion:(1) Prehypertensive group had higher HOMA-IR and BMI. The level of adiponectinwas reduced in the same group.(2) HOMA-IR and BMI as the independent risk factors may play an important role in prehypertension group. Adiponectin is an independent protective factor in the same people.(3) HOMA-IR as an independent risk factor may play an important role in stage 1 hypertension group.
