The System Operation Research Of The Urban Resident Basic Medical Insurance In Changchun

In recent years, the residents'demands to the medical and health service increase with the development of industrialization, urbanization and the aging of the population in China. It becomes a hot spot focused by the government, academician and civilization that how to solve the livelihood problem of"difficulty in getting medical service, expensive medical service". As the supplier of public health service, the government should changes its governmental functions and brings into play its leading role in order to provide basic and equal medical health guarantee. This is an inevitable option to improve the people's livelihood and comply to the will of people.As an important part of the medical care system of our country, medical insurance for urban residents is called"the troika of medical care system"together with the Urban Employees Basic Medical Care and the New Rural Co-operative Medical System. It is the key point to realize the nationwide medical insurance and build a harmonious society. The medical insurance for urban residents makes up the medical care system for the urban unemployed and solve the problem which the urban unemployed, especially the school students, children, the aged and the disabled, are difficult to take medical care. This is a landmark that our urban medical insurance system steps to a new historical phase. The document"Guideline about the Pilot Project of Urban Medical Insurance for Urban Residents"contains the 4-year target of taking 2—3 urban pilots in 2007, enlarging the pilots scope in 2008, increasing to more that 80% pilots in 2009 and finally roll out in nationwide. 2010 is the last year of the project. It is necessary to study the status of the system in the pilot cities.Firstly, this theme reviews the beginning and the development of the medical care and displays the status of the medical insurance system in our country. Secondly, on the base of new public service theory, market failures, public goods theory, with the leading role of the government in the medical care system of the urban resident, the theme identifies the responsibilities of the government in the area of system support, system propagation, system supervision, finance support and the relative infrastructure, etc. Taking the pilot example of Changchun, with the data by means of study, questionnaire and interview to the system operation status, the theme summarizes the experience and discovers the system defects. Furthermore, it proposes that the government should play an important role at the aspects of enhancing the attractiveness of the system, promoting the recognition of the system, reinforcing supervision, handing management of health care resources to achieve the optimal integration and pushing the reforms of the relative infrastructure of medical care system.
