The Study Of Raising The Orchestion Level Of China's Social Medical Insurance Fund

Social Security is not only a social policy being carried out in every countries all over the world today, but a basic civil right when a society has developed to a certain stage. Social security system plays a very important role in maintaining social stability. As an important part of the social security system and one of the country's main ways in revenue redistribution, its reform and development is the key to build a harmonious society and to improve the people's livelihood, and to effectively solving the "medical treatment is difficult and expensive" problem.State Council, "the recent focus on medical and health system reform plan" (2009-2011) explicitly requires specification of basic medical insurance fund management and improve the orchestration level of fund. Basic medical insurance fund for urban workers and residents'city (prefecture) orchestration should be basically realized. October 28, 2010, the 64th issue of "PRC Law on Social Insurance" was adopted on the 17th Meeting of The Eleventh National People's Congress, it provides that: "Progressively realize the basic endowment insurance fund at national orchestration level, and gradually implement other social insurance funds at the provincial orchestration level, the specific time and steps should be determined by the State Council". It is the first time that our country makes it clear in a legal form, so that the goals of the orchestration level of the social insurance funds become more clear and the improvement of the orchestration level for the fund have its legal basis. The promulgation of this series of policies and arrangements, especially "Social Insurance Law", pointed out the direction and proposed new requirements for building social medical insurance system with Chinese characteristics. It marked that the level of development of social medical insurance had achieved a big upgrade, marked that it had risen from county (district) orchestration level to city (prefecture) orchestration level, and would gradually shift to provincial orchestration level. It will promote the development of social medical insurance into a new historical stage.The fund orchestration level is the core of social treatment insurance management. In current situation, the rising of the fund orchestration level is not only targeted, but inevitable. But most domestic scholars focused on social insurance, especially on the orchestration level of endowment insurance fund, while studies on the orchestration level of medical insurance fund were relatively few, so, how to improve the orchestration level of medical insurance fund is the main content of this study. The whole article consists of eight parts. The 1st part is the introduction to the background, purpose, meaning of this study and to some views hold by some domestic and/or foreign scholars in this field; the 2nd part introduces the concepts and theories related to the orchestration level of social medical insurance fund; the 3rd part shows the historical development process and present situation of the social medical insurance system, it tries to find out the law of its development with analyzing the causes and courses to which the fund formed in a lower orchestration level; the 4th part shows the present situation and existing problems about the orchestration levels of social medical insurance fund for seeking solutions, and further discusses the necessity to improve the orchestration level; the 5th part introduces foreign and domestic exploration and practice on raising orchestration level of fund by citing cases such as China——Tianjin City, Hainan Province, Hunan Province, Guangzhong City and Germany, Japan, South Korea; the 6th part describes the path to improve the orchestration level of social medical insurance fund and the thinking on it, the path(develop a work scheme, make everyone's understanding consistent, advocate it and train the staff, build it systematically), the principles to be insisted (principle of low level, wide coverage and sustainable development, principle of not reducing reasonable treatments, ensuring smooth transition of the policy, principle of implementing scheme simply and effectively, principle of implementing scheme step by step in terms of insurance types, regions and steps) and the major problems need to be focused on solving (fund operation, management system, active work, information systems, and other problems, etc.) are more systematically discussed in this part; at last; the 7th part is the conclusion.To raise the orchestration of China's social medical insurance fund up to the provincial level, is not a simple task, nor a business or technical problem can be solved by investing, but an affair related to health care system reform, insurance fund transfer management, medical security level equalization and the vital interests of 1.25 billion insured Chinese people. It is impossible to be achieved within one day, or be passively delayed to a day that everything is ready before being implemented. We should advance the construction of orchestration through seizing the key items, implementing step by step, continuing to explore and practice and sum up experience based on the social situation of development, the people's needs and the basic conditions of work, seize the key, step by step, so that we can achieve the universal access to basic health care, universal access to basic health services, promote social fairness and justice, stability and harmony of the target.
