Study On The Implementation Issues Of Municipal Coordination Of The Basic Medical Insurance System

Based on international experience, building medical insurance system to provide universal health care, the key is system integration, and the path is simplification. The Chinese medical and health system reform has already entered a crucial stage. Both urban and rural health development should be developed, and overall level of health insurance should be improved. In fact, the urban and rural areas and improve the overall level are complementary because both are to coordinate health care management. But urban and rural areas should be explained as horizontal coordination of different administrative areas within a health care system, and the improvement of overall level is performed by integrating lower-level administrative units to higher ones. It requires unified medical insurance management throughout the system. Therefore urban and rural areas must improve the overall unification level of basic medical insurance system and it is also necessary to ensure the unification level. The municipal coordination is the route to promote the transition to provincial coordination. It is the necessary way to improve the overall level and also the route to expand health insurance coverage and increase the availability of health care services. It is also a key element to improve medical resource utilization, balance compensation across different regions, and resolve cross-region medical treatment and medical insurance transition problems.Therefore, this article accurately grasped the development direction of China’s long-term reform of the health insurance system. Based on a comprehensive analysis on the establishment of the medical insurance system and the overall level of related policies and regulations, this research clarified the coordination at provincial and municipal levels are the future reform trend. The research direction is so established:to use the theory of welfare economics as support, to combine it with China’s actual conditions, and to discuss the necessity of municipal coordination of basic medical insurance. Through field research of the implementation of the municipal coordination, the research selected Nanjing and Nantong as typical cities to specifically analyze the reform approaches. Based on results, this research tries to reach a universal conclusion from the individual attempts, and from existing pilot cities, the research gave a practical framework on policy standards, handling management, information systems, and operations of the fund. The framework proposed fund swap systems, regional municipal planning, and complete unified planning as three kinds of co-ordination model. From the analysis of the effect and implementation issues of municipal coordination of the basic medical insurance system, this research gives corresponding guidelines and policy advice in order to further improve and optimize the basic medical insurance system for the province.
