Objiective:Explore the anatomical basis of anesthesia in waist-shupoint by the study of applied anatomy of waist-shu point; Observe theadvantages and disadvantages of two kinds of anesthesia, and provide theguidance for the choice of anorectal operative anesthesia by the clinicalcomparative study of anesthesia of waist-shu point injection with traditionalChinese medicine and anaesthesia of waist-shu point,Methods:Understand and grasp the waist-shu point and its adjacentstructures and relationships by anatomising 4 sacrococcygeal specimens ofadult male cadavers, then record and analyse. Collect 80 cases by thestandards of diagnosis and treatment of diseases that need anorectal operation, inclusion criteria , exclusion criteria and exit criteria, and divide them intoexperimental group and the control group randomly (40 cases in each group):experimental group with 10ml of compound menthol (trade name: Ke Zepu)and 5ml of water for injection for anaesthesia of waist-shu acupoint-injection;control group with 20ml of 1.5% lidocaine for anaesthesia of waist-shuacupoint-injection. And observe side effects of anesthesia, pain during surgery, range of pain, degree of anal relaxation, pain after surgery, postoperative urineretention, edema of Anal margin, and compare advantages and disadvantagesof the two anesthesia methods by recording data and analyzing by statistics.Results:(1) The autopsy results confirmed that: there is a layer of fascia close to sacral dorsal ligament in sacral hiatus. Anatomise 4 sacrococcygealspecimens of adult male cadavers, and remove the skin, subcutaneous fat, posterior lobe of the thoraco-lumbar fascia, gluteal muscle, erector spinalmuscle, then see a layer of very thin fascia in sacral hiatus. Below the fascia(definition of the acupoints of the waist-shu acupoint location) there is a layerof connective tissue. The nerve in the sacral hiatus and in the place ofwaist-shu point definited by Acupoint Science by is encysted by the layer ofconnective tissue around a complete package. They look like a whole. We cansee the nerve after free some connective tissue. 2) analyze side effects ofanesthesia, pain after surgery, postoperative urine retention of the two groupsby statistics, P<0.05, the difference is statistically significant; and analyzepain during surgery, range of pain, degree of anal relaxation, edema of Analmargin of the two groups by statistics, P>0.05, the difference is notstatistically significant.Conclusion:(1) The autopsy results confirmed that: there is a layer offascia close to sacral dorsal ligament in sacral hiatus, The layer of fascia isconnected with the nerve around. We speculate that anesthesia effects ofwaist-shu point has some connection to the fascia and the adjacentconnective tissue. 2) Anesthesia of waist-shu point injection with traditionalChinese medicine has more advantages in terms of side effects of anesthesia, pain after surgery, postoperative urine retention by the clinical controlobservation.
