Objective To evaluate ringht ventricular funtion in patients with pulmonary embolism using Two-dimensional echocardiography,TDI and RT-3DE,and to explore the accuracy, sensitivity, superiority and clinical application of different methods in evaluating ringht ventricular funtion. Methods 28 patients (16 males and 12 females, mean age of 56.77±11.01years)with pulmonary embolism were divided into three groups as: high risk group(8 cases), moderate risk group (12 cases) and low risk group(8 cases),according to the expected PE-related early mortality.30 normal subjects(18 males and 12 females, mean age of 54.83±9.57years) were selected as control group. All subjects were undentaken Two-dimensional echocardiography,TDI and RT-3DE examination. Some traditional ultrasonic parameters of right heart,Tei index,RVEDV,RVESV,RVSV and RVEF were evaluted in all subjects. Application of SPSS 13.0 for windows statistical software to analyze, multiple groups were compared using analysis of variance, between groups were compared using the SNK test, inspection level a = 0.05.Results Compared with the control group, the right ventricular Tei index, RVEDV and RESV of high risk group of patients with pulmonary embolism was significantly increased (P<0.05),Their RVEF were all significantly decreased (P<0.05),They had characteristic echocardiographic appearance of right ventricular dilation and pulmonary hypertension. However, the Tei index, RVEDV,RESV and RVEF of moderate risk group also appeared significant difference (P<0.05),but had no dilated right ventricular. There was no significant difference in low risk group(P>0.05). And the Tei index, RVEDV,RVESV of moderate risk group was bigger than low risk group while smaller than high risk group;the RVEF of moderate risk group was smaller than low risk group while bigger than high risk group. Conclusion The right ventricular function of patients with pulmonary embolism can be normal, can also be damaged to varying degrees;Tei index measured by TDI acquired from tricuspid annular and RVEF measured by RT-3DE were objective, accuracy and sensitive indicators of right ventricular function.
