Objective:The aim of this study is to investigate the trends of the percentage of thyroid carcinoma(TC),including TC complicated with nodular goiter,TC with Hashimoto’s thyroiditis (HT),TC with thyroid adenoma(TA) and thyroid microcarcinoma. The diagnosis and treatment were influenced by the change of thyroid disease spectrum. By analyzed1837cases of TC which had treated by131I in our department,expecting to provide informations and directions for recognizing TC and associated thvroid diseases.Methods:The clinlical data of1837TC patients treated by131I in our department during recent8years were retrospectively analyzed. All cases were confirmed by intraoperative frozen section and postoperative pathological examination.There were1680cases of Papillary thyroid cancer (PTC) and157cases of Follicular thyroid cancer (FTC). The cases of before2000were one group,and after that year the cases were divided into five groups and every group includes two years, which checked the change of the percentage with chi square or chi square for trend analysis.Results:The incidence of TC with nodular goiter and that with HT progressively increase year by year. Compared with2000group,both the percentage of TC with nodular goiter in07-08,09-10and10-11(χ2=10.75, P<0.01; χ2=18.50, P<0.01;χ2=16.4, P<0.01) and that with HT in05-06,07-08,09-10and10-11(χ2=7.23, P<0.01;χ2=11.35, P<0.01; χ2=23.19, P<0.01; χ2=25.63, P<0.01) greatly increased. The incidence of TC with TA was raised up before05-06,but it began to descend after those years. Compared with05-06group,the incidence of TC with TA in09-10and10-11group were significantly decreased(x2=8.32, P<0.01;χ2=7.67, P<0.01). The high incidence of TC patients that both men and women were all the age of36-55. TC was about three times more frequent in women than in men before the age of25,but the sex ratio was dropped with the increasement of age. Compared with the age of4-25,the sex ratio of TC droped significantly in the group of56-65and66-82(x2=6.99, P<0.01; χ2=8.29, P<0.01). The incidence of TMC was raised up, Compared with2000group, the percentage of TMC in07-08,09-10and10-11(p<0.01) greatly increased. In addition,45.28%cases were accompanied with benign thyroid nodular.Conclusions:Both the coexistent rate of TC with nodular goiter and that with HT greatly increase recently,while that with TA tends to decrease. TMC has high-level morbility and accompanies common benign thyroid nodular mass.
