Objective:This study was aimed to observe the co-expression levels of the platelet membrane proteins CD36, CD63together as well as the plasma von Willebrand factor (vWF) in acute cerebral infarction (ACI) patients, and to explore their asoccisiations with ACI. In addition, it was detected whether to monitor the degree of platelet activation via the expression levels of CD36, CD63and vWF to predict the ACI disease progression and prognosis, and to provide a new theoretical basis for the ACI.Methods:According to the diagnostic criteria revised by the4th China Cerebrovascular Disease Conference, this study enrolled39ACI patients,40patients in the convalescent stage of cerebral infarction and38healthy adults as a normal control group. The co-expression of the platelet membrane protein CD36and CD63was detected by flow cytometry in all patients and the control group enrolled. And the level of vWF in plasma was measured by enzyme-linked immunosorbent assay (ELISA). Then the correlations were analyzed between the different risk factors of cerebral infarction and the co-expressin levels of CD36/CD63and vWF. Correlation coefficients were calculated to examine the relationships between the co-expression levels of CD36/CD63, vWF and vascular risk factors of Cerebrovascular Disease as well as prognostic values of CD36/CD63, vWF to the former diease.Result:1. The double immunomagnetic positive co-expression of the platelet membrane proteins CD36, CD63and the plasma vWF levels dramatically increased in ACI patients than those in the control group.2. The double immunomagnetic positive expression levels of CD36and CD63showed significant line positive correlation with neurologic impairment score in ACI patients. As to the infarct volume of the ACI patients, the double immunomagnetic positive expression levels of CD36and CD63in platelets and the level of vWF were higher in the large volume group than that of the medium or small size group. This results indicated that platelet activated degree can reflect the pathogenetic condition.3. The double immunomagnetic positive expression levels of CD36/CD63and vWF expression were significantly increased in ACI patients than those in the convalescent stage of acute cerebral infarction. It suggested that the CD36/CD63and vWF can be used as the indexes for occurrence, development and pathogenetic condition changes of acute cerebral infarction.4. The double immunomagnetic positive rate of platelet CD36/CD63in ACI patients was positively correlated with the low-density lipoprotein, triglycerides, cholesterol and neurological deficit scores. Compared with CD36/CD63, there were no significant correlations between the vWF and the above measured clinical factors. This data showed that in addition to platelet, the high co-expression levels of CD36/CD63might affect other risk factors for stroke.5. The plasma vWF concentration was positively correlated with CD36/CD63co-expression, and these findings indicated that double immunomagnetic positive rate of CD36/CD63increased as the plasma vWF concentration raised.Conclusion:Platelet activation plays an important role in the process of acute cerebral infarction. This study found that co-expression of CD36and CD63detected by FCM can be used as sensitive indexes of platelet activated degree. The CD36and CD63co-expression level was positively correlated with progression of ACI. They can influence the prognosis of cerebral infarction. The plasma vWF concentration and the double immunomagnetic positive rate of platelet CD36/CD63also contributes to the occurrence and development of acute cerebral infarction. Plasma vWF concentration may be considered as one of the risk factors of cerebral vascular disease. The plasma vWF concentration was significantly correlated to the double immunomagnetic positive rate of platelet CD36/CD63. These evidences support that causal relevance exists between cerebral infarction and the plasma vWF concentration by its influence on platelet activation.
