ObjectiveThe acne is one kind of hair-follicle sebaceous glands chronic inflammation disease which is caused by various factors, it is a skin illness which is common and muitiple during the puberty and adulthood. The Chinese medicine calls it "pneumonia wind acne". Pricking blood therapy is one of traditional acupuncture methods which punctures certain acupoints, the infection place, the pathology responsive loci or superficial unusual venule and sheds a certain amount of blood to treat disease by three edged needle, plum-blossom needle, the acupuncture needle, entry needle or other tools. The effects of pricking blood therapy is achieved by harmonizing qi and blood in order to regulate the yin and yang, to dredge the channels, and to strengthen the healthy and dispel the pathogenic. In this clinical study, we adopt randomized controlled study to assess the feasibility, effectiveness and security of pricking blood therapy on facial acne with the pathomechanism of stasis heat and stagnant phlegm, which is in order to provide a reliable basis for larger samples in clinical studies and provide a new thinking and a new method to the clinical study of facial acne with the pathomechanism of stasis heat and stagnant phlegm.MethodsWe select60standards-compliant facial acne with the pathomechanism of stasis heat and stagnant phlegm patients, and divide them into two groups averagely which are a pricking blood treatment group and a acupuncture therapy control group. One group take a treatment as follows:1. Pricking blood treatment groupStabbing blood prescription:Positive reaction spots nearby Du meridian and bladder meridian of Taiyang on the back, includind the stasis venation, the tender area, hypodermic tubercle, the acne pus blister, the expanding thick skin pore.Manipulations:Sting directly at the positive reaction spot on the back with disposable injector pinhead after routine disinfection, click out the needle quickly and let blood spill out naturally; If the blood runs unsmoothly, put a fire cup on it to straw the blood out, choose2~3positive reaction spots for one time and the amount of bleeding should be controlled within about1~2ml. Stings twice perweek,4weeks makes a treatment course.2. Acupuncture therapy control groupAcupuncture prescription:Hegu, Quchi, Shangxing, Taiyang, Yangbai, Dazhui, Xuehai, Fenglong.Manipulations:Treat all the spots above except xuehai with reducing method in acupuncture, and treat Xuehai with mild reinforcing-reducing method, retaining the needle for30minutes each time. Acupuncture twice perweek,4weeks makes a treatment course. All patients will treat for one month and follow up for two weeks. We use the acne efficacy evaluation table and the TCM syndromes scale of two groups before and after for efficacy evaluation.ResultsThrough this study, we sum up the following results:1. After1week treatment:①Two groups are ineffective to improve the acne symptom(P>0.05).②The treatment group is effective in pre therapy and post-treatment compared to the TCM syndromes scale (P<0.05), and the control group is ineffective in pre therapy and post-treatment compared to the TCM syndromes scale (P>0.05).③The effect of treatment group is better than the control group in improving TCM symptom.2. After one course of treatment:①Two groups are both effective to improve the acne symptom and TCM symptom(P<0.05).②According to statistics, The effect of treatment group is better than the control group in improving acne symptom and TCM symptom(P<0.05).3. After treatment, the results show that the total effective rate of the treatment group is96.67%while the total effective rate of the control group is86.67%. The treatment group’s effect is better than the control group.4. Two weeks after the treatment termination:According to the statistically significant results, there is no significant difference of the treatment group in both acne symptom and TCM symptom (P>0.05), while the control group has differences in acne symptom and TCM symptom (P<0.05). The results show the long-term efficacy and the sustained efficacy of the treatment group last longer than the control group.Conelusions1. Pricking blood therapy has certain effect in the treatment of facial acne with the pathomechanism of stasis heat and stagnant phlegm patients. This therapy is an effective therapy in improving acne symptom and TCM symptom, and it is effective, convenient, safe.2. Pricking blood therapy’s efficacy is rapid in the TCM physical improvement, and lasts longer than acupuncture group. ALL in all, it’s effective and has better long-term effects and obvious physical improvement.
