Purpose:combination of treatment based on syndrome differentiation of traditional Chinese medicine theory, based on the theory of modern medicine and science and technology. In intercellular adhesion molecules (ICAM-1) affect inflammation, thus improving local inflammatory response in diabetic foot, to discuss Zhao’s "Tonifying and Elimination Low" treatment of diabetic foot ulcer (or gangrene), improving the ICAM-1role between the factors associated with a local inflammatory reaction. And to inherit and carry forward Zhao’s "Tonifying and Elimination Low" thoughts on diagnosis and treatment of diabetic foot treated, medicine mining heritage, so that will bring more and more benefits to patients who with diabetic foot.Methods:a randomized clinical study met the inclusion criteria of39patients with diabetic foot and6health person, divided into four categories. Group name:pure herbal medicine group, herbal and western medicine group, pure western medicine group, control group. And giving the groups the same required clinical diabetes in diabetic foot patients’treatment. Respectively to the four groups on this basis drugs:pure herbal medicine group (13cases):take orally Zhao’s Prescription and Zhao’s "herbal cream"for external, herbal and western medicine group (12cases):take orally Zhao’s Prescription and "Rivanol"(Ethacridine Lactate Solution) for external, pure western medicine group (14cases):take oral ly "Pentoxifyl line control led release tablets"(0.4g/pill) and "Rivanol"(Ethacridine Lactate Solution) for external, control group (6cases):just giving the only clinical general treatment of diabetes for patients, not additional medication. Observation of the measured value of ICAM-1by the groups before and after treatment. Using SPSS18.0statistical software, data a mean±standard deviation of each (x±s) said that using rank sum test to compare efficacy in the three groups, using t test on three general comparisons in groups.Results:by observation of clinical trials, each group of ICAM-1comparison, came to the following conclusion:no significant differences between treatment groups, involved in the control of herbal and western medicine in three groups, there are significant differences before and after treatment, namely:group before and after treatment of herbal medicine:P=0.000<0.001, group before and after treatment with herbal and western medicine:0.01>P=0.005>0.001, western group before and after treatment:P=0.013<0.05; After treatment, no significant differences among the three groups (P>0.05), and also no significant differences between the normal control group, respectively (P>0.05). After treatment, no significant differences among the three groups. Instructions after treatment by the group ICAM-1all have different degrees of decline, but clinical drug trials and no significant differences among the three groups.Conclusions:through clinical trials, by Zhao’s "Tonifying and Elimination Low" derived Zhao’s Prescription and Zhao’s "herbal cream", have a certain effect on the treatment of diabetic foot gangrene. ICAM-1has a different degree of decline, but no significant difference among the three groups. Hints of traditional Chinese medicine and Western medicine on ICAM-1has a role in controlling, might pass on the regulation of ICAM-1, thereby reducing the adhesion of leukocytes and endothelial cells, to extent control the inflammatory response occurs. Because the wound infection of foot and all patients used antibiotic, determination of a degree of influence on ICAM-1. Later, Zhao’s "Tonifying and Elimination Low" in control on the inflammatory response, a certain degree of influence, but it is not significant.
