| Objective: The goals of this study were to explore the "cocktail" treatment repair the epilepsy patients’hypoperfusion foci.The paper also discussion the change of the long-term V-EEG.Methods: The present study was performed in40epilepsy, who were diagnosed at the Second Hospital of Lanzhou University, Epilepsy Center from February2011to July2011. They all have hypoperfusion focal based on the inclusion and exclusion criteria. We clearly record patients, the general condition, determined the seizure type, and investigated their findings of interictal SPECT, V-EEG and CT/MRI. On the base of taking the original antiepileptic drug, moreover,(1) TMP;(2) Edaravone injection;(3)Citicoline Sodium Injection;(4)Magnesium Sulfate Injection. A period of treatment for14days,then, telephone follow-up for3months to evaluate the treatment effect. Finally, we analyzed the sensitivities and specificities values of interictal SPECT, V-EEG and CT/MRI by SPSS17.0statistical software. Results:In the study cases, male29cases, female llcasea, the sex ratio is2.6:1.The age during4to61, the median age is17.5years. The course of disease from13months to51years, the median is5yeas. Seizure types included simple partial seizure4cases, complex partial seizures1case, simple partial seizure secondary generalized tonic clonic seizures4cases, complex partial seizures secondary generalized tonic clonic seizures4cases, generalized tonic clonic seizures18cases, Temporal lobe epilepsy5case, Frontal lobe epilepsy1case, the Benign epilepsy of childhood with centrotemporal spikes(BECT)1case, Lennox-Gastaut syndrome2cases.The CT/MRI,normal30cases (75%),abnormal10cases (25%), include hippocampal sclerosis5cases, perencephaly1case, dysplastic gangliocytoma of the cerebellum (Lhermitte-Duclos disease)1case, encephalatrophy1case, encephalomalacia2cases. The changes of interictcal SPECT before and after treatment: single foci17cases (42.5%), multifoci16cases (40%), extensive hypoperfusion foci7cases (17.5%);mildly abnormal9cases (22.5%), moderately abnormal16(40%), severely abnormal15(37.5%).After treatment, single foci6cases (15%), multifoci9cases (22.5%%), extensive hypoperfusion foci1cases (2.5%);mildly abnormal5cases (12.5%), moderately abnormal3(5.0%), severely abnormal9cases (22.5%). Compared interictcal SPECT before and after Cocktail treatment, normal increase in24cases (60%), at the number of lesions, single foci cases reduce11cases, multifoci cases reduce in7cases, extensive hypoperfusion foci cases reduce6cases. The positive rate had significant difference between before and after interictal SPECT (x2=35.721, P=0.000<0.01). From the degree, mildly abnormal cases reduce4cases,moderately abnormal cases reduce in12cases, severely abnormal cases reduce6cases.The positive rate had significant difference between before and after cocktail treatment in interictal SPECT (χ2=37.523, P=0.000<0.01). The changes of V-EEG before and after cocktail treatment: normal was in12cases (30.0%), abnormal in28cases (70%), include limitation rate in15cases (37.5%), universality13cases; After the treatment of cocktail, the cases of normal increased11cases, the abnormal cases include limitation11cases (27.5%) and universality6cases (15%). The positive rate had significant difference between before and after cocktail treatment in V-EEG (χ2=6.651, P=0.036<0.05). This study of all cases by telephone follow-up after discharge, its curative effect for8cases(20%) were cured,10cases (25%) were markedly improved, the improved case in12cases,and8cases (40%) failed.Conclusions:"Cocktail" therapy can be significant repair epilepsy patients with hypoperfusion foci, and at the same time "cocktail" therapy is a important aspect in the epilepsy treatment. |
