Objective To observe the effect on PVP-I combination of salvianolate to the postoperative intestinal adhesion grade in rats model of intestinal adhesion and TNF-a, IL-6expression level in blood serum, and to explore its functioning mechanism.Method Selection of clean and healthy level of88SD rats, half male and half female, were randomly divided into5groups:normal control group (n=8), model control group (n=32, according to1,3,5,7days of4time points evenly distributed), PVP-I group(n=8), salvianolate group (n=8) and PVP-I combined with salvianolate group (n=32, allocation method with model group). Except normal control group, intestinal adhesion modeling should be conducted to other rats according to Hugh. Abdominal closure suture a final dosing, normal control group and model control group of intraperitoneal injection of5%glucose injection4ml/kg, The treatment group of PVP-I group, salvianolate group and combination group were intraperitoneally at concentrations of0.625%(based on the earlier experimental experience indicates that this concentration is better) solution of PVP-I4ml/kg、salvianolate solution of10.9mg/kg and the mixture solution of above concentration PVP-I4ml/kg and salvianolate solution10.9mg/kg. And on the first postoperative day continuous injection of7days,l times/day. The model control group and combination group after operation respectively1,3,5,7days, other groups are respectively on the seventh postoperative day in U shaped incision open during narcosis, according to the Nair classification standard, observed and assessed intestinal adhesion grade in each group by the naked eye; Immediately, sampled, anti-coagulated and centrifugated the blood of postcava to obtain the blood serum, and preserved it at an ultra-low temperature. A later ELISA method would be employed to test the concentration of TNF-a and IL-6in the blood-serum samples. Sampled the adhesive tissues from the most obvious part with the size of1.0cm×1.0cm×0.5cm, and placed the samples in the10%neutral formaldehyde fixative for24hours. Embed the samples in the paraffin, made slices, conducted the HE coloration, and observed expression of fibroblast and inflammatory cells by microscope observation.Result1. intestinal adhesion:the adhesion degree of model control group was higher than the control group (P<0.01); salvianolate group and combination group were significantly lower than that of model group (P<0.01); PVP-I group was lower than the model group, but there was no statistical significance (P>0.05); the combination group of intestinal adhesion degree lower than that of group PVP-I and salvianolate group, but there was no statistical significance (P>0.05);2. After seventh days of serum TNF-a level of expression:compared with the normal control group, model control group was significantly higher than that of the normal control group(P<0.01); compared with model control group, PVP-I group, salvianolate group and combination group were significantly lower than that of model group (P<0.05or P<0.01); PVP-I group and combination group compared with statistical significance (P<0.05); there was no statistical significance of salvianolate group and combination group (P>0.05);3. After seventh days of serum levels of IL-6expression: compared with the normal control group, model control group was significantly higher than that of normal control group (P<0.01); compared with model control group, PVP-I group, salvianolate group and combination group were significantly lower than that of model control group (P<0.05or P<0.01); PVP-I group and combination group compared with the obvious statistical significance (P<0.01); salvianolate group and combination group compared with statistical significance (P<0.05);4. At each time point after operation of rat serum TNF-α dynamic change:the serum TNF-a level of model control group from the first postoperative day gradually increased, on the seventh postoperative day reached the highest; the combination group TNF-a level on the fifth postoperative day to reach the highest, and the seventh days of decline. Two groups of all time points corresponding comparison, there were significant differences in the two time points of the fifth and the seventh days (P<0.01);5. At each time point after operation of rat serum IL-6dynamic changes:the serum IL-6level of model control group from the first postoperative day gradually increased, on the seventh postoperative day reached the highest; the combination group IL-6levels on the seventh postoperative day reached the highest. Two groups of all time points corresponding comparison, there were significant differences in the three time points of the third、the fifth and the seventh days(P<0.01or P<0.05);6. Light microscopic observation of the model control group and the treatment group pathological section, the result shows compared with model control group, PVP-Ⅰ group, salvianolate group and combination group of intestinal adhesion tissue fibroblasts proliferation and infiltration of inflammatory cells was reduced.Conclusion Cytokine IL-6, TNF-α in intestinal adhesion formation play a certain role; PVP-Ⅰ, salvianolate, PVP-Ⅰ combined with salvianolate can reduce postoperative intestinal adhesion degree, and reduce the TNF-α and IL-6expression levels, and the combination group effect is more obvious.
