Background and Objective:The interaction between the genetic and environmental factors lead to thedevelopment of Type2diabetes and its complications. Receptor for advnaecdglyaction endporudcts(RAGE) plays an important role. in diabetic vascularcomplications.This study was to investigate the relationship between2184A/Gpolymorphism of RAGE gene and diabetic microvascular complications (includingdiabetic nephropathy and diabetic retinopathy) in chinese han patients with type2diabetes.Methods:626patients with type2diabetes(T2DM) and128subjects with normal glucosetolerance(NGT) were enrolled in this study. According to24hours microalbuminuria,serum creatinine and endogenous creatinine clearance,the T2DM patients weredivided into three groups: without nephropathy group(DN-0), incipient nephropathygroup(DN-1) and clinical nephropathy group(DN-2); According to theretinopathy,,the T2DM patients were also divided into two groups: withoutretinopathy group(NDR) and with retinopathy group(DR). The genotype and allelefrequencies of polymorphism2184A/G in RAGE gene were detected by using thepolymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)method.The data was analysized by SPSSl7.0statistical software.Results:1. There were two alleles(A and G) and three genotypes(AA, AG, GG) ofpolymorphism2184A/G in NGT group and T2DM group.There were no statisticallysignificant differences in genotype and allele frequencies of polymorphism2184A/Gbetween NGT group and T2DM group(P>0.05).2. There were gradually decline trend in frequencies of the genotype carrying Galleles (AG+GG) and the G allele among DN-0group,DN-1group and DN-2group (trend2=4.796, P=0.029). The frequencies of the genotype carrying G alleles (AG+GG) and the G allele in DN-2group were statistically significantiy higher thanthose in DN-0group(2=4.408, P=0.036).3. The frequencies of the AG+GG genotype and the G allele of polymorphism2184A/G in DR group was a little lower than those in NDR group, but the differencesbetween the two groups were not statistically significant(P>0.05).4. There were no tatistically significant differences in age, gender,known-duration of diabetes, HbAIc, FCP,2hCP, TC and TG,HDL-c, LDL-C, SBPand DBP between T2DM patients with the genotype carrying G alleles (AG+GG)group and with AA genotype group. However, the levels of Scr and BUN in the AG+GG genotype group were statistically significantly lower than those in AA genotypegroup(P<0.05).Conclusions:1. Polymorphism2184A/G of RAGE gene is related to the development ofdiabetic nephropathy in Chinese Han nationality patients with type2diabetes. the Gallele may be protective factor for type2diabetic patients from diabetic nephropathy.2. Polymorphism2184A/G of RAGE gene is not related to diabetic retinopathyin Chinese Han nationality patients with type2diabetes.
