Objective:To investigate the role of serum vascular endothelial growth factor(VEGF) andestradiol(E2) and urinary basic fibroblast growth factor(bFGF) in determining thestages of infantile hemangiomas and differentiating it from vascular malforma-tions,and explore the mechanism of propranolol in treatment of proliferative hemang-ioma by observing the variation of serum VEGF.Methods:138patients with infantile hemangiomas or vascular malformations whichheaded for the Dermatological department and Paediatrics of the first affiliatedhospital of Nanchang university from January2011to March2012were involved,including60proliferative hemangiomas,38involuting hemangiomas and40vascularmalformations,and2.5milliliter venous blood was drew on an empty stomach while5milliliter early morning urine was gathered. And8patients with proliferativehemangioma were treated with oral propranolol. ELISA was used to determine serumVEGF and urine bFGF,while E2was detemined by electrochemiluminescence assay.Results:1) The serum VEGF and estradiol and urinary bFGF concentrations ofproliferative hemangioma group were significantly higher than involutinghemangioma group, vascular malformation group and the normal control group(all P<0.001).2)The difference of serum VEGF and urinary bFGF between involutinghemangioma group and vascular malformation group and the normal control groupshowed no statistical differences(all P>0.05),the serum estradiol level was higherthan vascular malformation group and the normal control group,which showedstatistical significance(all P<0.05).3)The serum VEGF and E2concentrations inproliferative hemangioma group and involuting hemangioma group showed nodifferences in different sexs(all P>0.05).4)There were no statistical relevancebetween the concentrations of the serum VEGF and estradiol and urinary bFGF in proliferative hemangioma group and involuting hemangioma group.5)Serum VEGFof8proliferative hemangiomas at2weeks and6weeks after treatment with oralpropranolol decreased gradually,and at6weeks after treatment was significantlylower than that before treatment.Conclusions:The detection of serum VEGF and E2and urine bFGF were simple, easy andreliable, which can be used to differentiate hemangiomas from vascularmalformations and provide the basis for the clinical stages of infantilehemangiomas.Propranolol is effective for the treatment of proliferativehemangioma,possibly by reducing the concentration of serum VEGF.
