Objective:To evaluate the expression of protein P1GF with its receptor Flt-1in gastric carcinoma and detect the variation of micro vessel density marked by CD34in tumor tissues and normal tissues and investigate their relationships with tumor infiltration, metastasis, prognosis and clinicopathological character of gastric carcinoma.Methods:A series of84specimens of human gastric adenocarcinoma that had undergone gastrectomy were collected in Qilu Hospital of Shandong University from2006to2007while20specimens of adjacent normal gastric tissues as control. Immunohistochemical technique was used to examine the expression of P1GF, Flt-1protein and MVD, which was calculated by the mark of CD34, and their relationships with tumor infiltration, metastasis and prognosis. COX proportional regression model was used to define the prognostic factors that determine the survival of the patients.Results:In gastric cancer tissues, P1GF mainly localizes in the cytoplasm of gastric cells and (or) cell membrane, the positive rate was78.6%(66/84); but the adjacent normal tissues was seen weak positive staining and the positive rate was5%(1/20). The expression of P1GF in gastric tissues was significantly higher than that in adjacent tissues (P<0.05). The positive expression of P1GF was closely associated with differentiation, invasion depth, Borrmann type, lymph node metastasis (P<0.05) while it had no significant correlation with sex, age, tumor size and location (P>0.05). In gastric cancer, Flt-1mainly localizes in the cytoplasm of gastric cancer cells; the positive rate was67.9%(57/84), while in the adjacent normal tissues Flt-1was negatively expressed. The positive expression of Flt-1was associated with invasion depth and Borrmann type (P<0.05), and it had no significant correlation with sex, age, tumor size, location, differentiation and lymph node metastasis (P>0.05). The rate in high MVD expression group was59.5%(50/84), and MVD was associated with invasion depth, Borrmann type, lymph node metastasis (P<0.05), and it had no significant correlation with sex, age, tumor size, location and differentiation (P>0.05). The expressions of PIGF and Flt-1and that of PIGF and MVD were positively correlated, but the expression of Flt-1and MVD was negatively correlated. The survival time in patients with positive expressions of PIGF and Flt-1was lesser than those with negative expressions (P<0.05). The Kaplan-Meier curve indicated that PIGF and Flt-1were related with prognosis in gastric cancer. By COX multivariate analysis, the invasion depth and Flt-1expression were found to be the independent prognostic factors of gastric cancer patients (P<0.05)Conclusions:1. PIGF is significantly high expressed in gastric cancer, which is associated with differentiation, infiltration, Borrmann type and lymph node metastasis; Flt-1positive expression rate is also very high, and there is no positive expression in normal tissues. It is associated with infiltration and Borrmann type. Their high expressions are positively related in gastric cancer. It indicates that PIGF and Flt-1play an important role in invasion and metastasis of gastric cancer.2. The survival time of patients who underwent operation with gastric cancer is affected by the expression level of PIGF and Flt-1, and patients have long survival time with PIGF and Flt-1negative expression.3. Flt-1and infiltration are independent factors which affect the prognosis of gastric cancer, Flt-1negative expression tips a long survival time. Therefore, it may valuable that Flt-1is used for the prognostic estimation of gastric cancer.4. Detection of PIGF and Flt-1favors to the judgment of tumor infiltration, metastasis in gastric cancer and can evaluate the grade malignancy and prognosis. Combined with clinical material, it may be used for clinical diagnosis of gastric cancer and prognosis as one of the auxiliary indexes.
