ObjiectiveInvestigate the expression of thrombin-activatable fibrinolysis inhibitor (TAFI) in cervical squamous cell carcinoma tissues with and without pelvic lymph node metastasis and explore its clinical significance.Subjects and methods1.Subjects:17cervical squamous cell carcinoma patients with pelvic lymph metastasis and23ones without metastasis were enrolled in the study.All patients had undertaken radical hysterectomy and pelvic lymphadenectomy.Tissue samples were collected and coagulation tests were taken before treatment,including:fibrinogen degradation products(FDP),prothrombin time(PT),activated partial thromboplastin time (APTT), thrombin time(TT),fibrinogen(Fbg),D dimmer(D-D),antithrombin III(ATIII).2. Evaluate the relationship between pelvic lymph node metastasis of cervical squamous cell carcinoma and coagulation parameters and clinico-pathologic parameters.3. Detect expression of TAFImRNA and its protein in cervical squamous cell carcinoma tissues of the two groups with application of Fluorescent real-time PCR and Western blot technique,and further analysis the relationship between expression level of TAFI and pelvic lymph node metastasis and clinical-pathologic parameters.4. Statistical Analysis:All statistical analyses were performed using the SPSS17.0program. A p value<0.05was considered statistically significant.Results1. Results of analysis of coagulation parameters and clinical-pathologic parameters in two groups(1)Plasma concentration of D dimmer was elevated in metastasis group compared the control(P=0.005),while the other coagulation parameters(FDP,PT,APTT,TT,Fbg,ATⅢ)showed no significant difference between the two groups (P>0.05)(2)Vessel involvement, depth of muscular invasion and abnomal plasma level of D dimmer were associated with pelvic lymph node metastasis of cervical squamous cell carcinoma (P<0.05),while age,clinical stage,tumor size,grade of differentiation showed no significant relevance with nodal metastasis (P>0.05)2. Results of Real Time PCR(1) Expression of TAFImRNA in cervical squamous cell carcinoma tissue was ranged from0.000036to0.035550, and the mean was0.002200±0.005905.(2) TAFImRNA expression was elevated in metastasis group compared the control(0.004238±0.008748vs.0.000694±0.000754) (P=0.035).TAFImRNA expression showed no significant relevance with vessel involvement,depth of muscular invasion and plasma level of D dimmer (P≥0.05)2. Results of Western blot(1)TAFI protein expression in cervical squamous cell carcinoma tissues was ranged from0.507~2.204,and the mean was0.834±0.286.(2) TAFI protein expression was elevated in metastasis group compared the control (1.002±0.337vs.0.710±0.155)(P=0.003).TAFI protein expression was associated with vessel involvement and plasma level of D dimmer(P<0.05).There was no significant association between TAFI protein expression and depth of muscular invasion (P>0.05)ConclusionTAFImRNA and protein expression in cervical squamous cell carcinoma tissues were up-regulated in pelvic lymph node metastasis group compared negative metastasis group.Furthermore,up-regulated expression of TAFI protein in cancer tissues was associated with vessel involvement and abnormal plasma level of D dimmer. It indicated that up-regulated expression of TAFI protein in cervical squamous cell carcinoma tissues played a role in pelvic lymph node metastasis and hypercoagulable state.
