Object i ve:By the research of using herbal compound preparations (converging essence and expelling turbidity, dispersing blood stasis and clear collaterals) combined with Western medicine Los Dingxin, observes the clinical curative effect of this method for treating early stage of Diabetic Nephropathy (DN). It is aimed at finding more effective treatment to delay the occurrence and development of DN, so as to ease the clinical symptoms of the patients and improve their life quality.Methods:Referring to the redesigned Diabetes Diagnosis Standard of WHO in1999, corresponding with the diagnosis of Type II Diabetes mellitus (TIl DM), and according to the internationally recognized staging standards of MogenesenDN, we choose the60hospitalized patients with I-III Diabetic nephropathy (early stage of DN) in Department of Diabetes and Nephropathy in Affiliated hospital of Shanxi College of TCM, which their Syndrome differentiations of TCM pertain to the deficiencies of Qi and Yin, blood stasis stagnating the collaterals. It is divided into two groups using the randomized and compared method:30in treatment group (herbal compound preparations plus Los Dingxin) and30in the control group (Los Dingxin). Basic treatment on the patients of two groups, we choose suitable hypoglycemic agent according to their own individual situation in order to control their blood glucose in ideal range: Fasting blood glucose (FBG)≤7. Ommol/L, Postprandial blood glucose (PBG)<10. Ommol/L. On this base, the control group take Los Dingxin500micrograms orally, twice each day; the treatment group take herbal compound preparations of converging essence and expelling turbidity, dispersing blood stasis and clear collaterals besides Los Dingxin. Observing indexes:1. Observe the two groups separately of their change on main clinical syndromes and signs before and after the treatment, calculate syndrome integral of TCM and compare their curative effect after statistics processing.2. Observe the two groups separately of their blood glucose (FBG, PBG, HbAlc), blood lipid (CHO, TG), hemorrheology (whole blood high, medium, low viscosity), microalbuminuria (MAU) discharge and24hours urine protein (UP) ration before and after the treatment and compare their curative effect after statistics processing.Results:The clinical research shows that the treatment group has obverse advantages over improving their clinical syndromes and signs. The total efficiency is93.34%,40%higher than that of the control group. After treatment, FBG, PBG, HbAlc of treatment group are reduced compare to these indexes before the treatment, but in interclass and intraclass there are no statistic significances. After treatment, by the comparisons of CHO, TG, the intraclass of the two groups has statistic significances (P<0.05) and the interclass also has statistic significances (P<0.05). By the comparisons of hemorrheology after the treatment, the interclass of the treatment group has significant differences (P<0.01) and intraclass of two groups also has significant differences (P<0.01). By the comparisons of microalbuminuria (MAU) discharge and24hours urine protein (UP) ration after the treatment, the interclass has significant differences (P<0.01) and the intraclass has statistic significances (P<0.05) that the numerical decline range of the treatment group is higher than that of the control group.Conelusions:On the basis of conventional treatment of using western medicine of Los Dingxin, plus herbal compound preparations of converging essence and expelling turbidity, dispersing blood stasis and clear collaterals, can not only improve the patients’ clinical syndromes and signs of early stage of DN obviously, but also have certain effect on controlling blood glucose and regulating blood lipid. Besides, it can reduce microalbuminuria (MAU) discharge and24hours urine protein (UP) ration to some extent. From the above, it is confirmed that the methods of converging essence and expelling turbidity, dispersing blood stasis and clear collaterals on treating the early stage of DN have great advantage compare to Western medicine. |
