Objective Based on stakeholder theory and game theory, this paper carries out stakeholder research on National System for Basic Pharmaceuticals, takes the definition, classification of stakeholders of National System for Basic Pharmaceuticals, quantifies its properties and effects. This paper also discusses the game behavior of core stakeholders, builds benefit models between many stakeholders, analyzes how to achieve the benefit equilibrium of National System for Basic Pharmaceuticals, in order to provide policy basis to promote the sustainable development of National System for Basic Pharmaceuticals.Methods On the basis of literature review, this paper uses the method of Delphi, personal interview and field survey and so on to research on stakeholders. Combined with theoretical analysis, data analysis and model analysis, this paper explores benefit equilibrium of National System for Basic Pharmaceuticals.Results (1) Stakeholders of National System for Basic Pharmaceuticals includes government, medical institutions, pharmaceutical production and circulation enterprises and community residents. They are a total of four categories,17kinds. Stakeholders attribute for power=0.44, legitimacy=0.33, urgent=0.23. Six kinds of stakeholders such as health departments belong to definitive stakeholders, seven kinds of stakeholders such as community health service institutions belong to the expected stakeholders, four types of stakeholders such as community residents belong to the latent stakeholders. The overall stakeholder impact index of stakeholders of National System for Basic Pharmaceuticals is1.93, among them, health departments have the highest impact index which is0.43.(2) Compared to the local government,central government’s health investment is higher, its annual investment proportion is72.82%.(3) Economic operation situation of community health service institutions affected by National System for Basic Pharmaceuticals, basic drug use behavior is improved, community health service institutions’effort level related with the government’s investment and the level of compensation.(4) The rent-seeking model shows that basic drug bidding and purchasing has the possible of rent-seeking, medicine production and circulation enterprises may seek improper benefit by rent-seeking activities.(5)58.3%of the community residents surveyed choose pharmacy to buy drugs,32.1%of residents choose community health service institutions. Community awareness of National System for Basic Pharmaceuticals and demand for basic drugs is low, only12.9%of residents know t National System for Basic Pharmaceuticals, only0.3%of the residents think basic drugs can meet the demand very much But54.6%of community residents are less familiar and unfamiliar with National System for Basic Pharmaceuticals,27.3%of community residents’ demand is small and very small.Conclusions The stakeholders of National System for Basic Pharmaceuticals have different benefit, their impact on the public policy is different, National System for Basic Pharmaceuticals can get better support of stakeholders. National System for Basic Pharmaceuticals’ benefit equilibrium should maximize the four stakeholders benefit, it should strengthen the government’s leading role, arouse the role of community health service institutions, standard the pharmaceutical production and circulation enterprises’ role, meet the community residents’ medical needs. So for coordinating stakeholders’ benefits, making up the benefit equilibrium, promoting the stable development of National System for Basic Pharmaceuticals, it should take the government as leader, strengthen incentive and supervision, and make up the community health service institutions’ finance balance, regulate the behavior of pharmaceutical production and circulation enterprises, intensify publicity of National System for Basic Pharmaceuticals and improve the awareness of community residents.
