The aging of population has become a global problem,thepopulation of China has become a key issue in recent years.Thehealthy and happy life of the elderly reflect the social civilization.Providing health care services is a new challenge in China.In order tosolve the problem of the survival and development of millions elderlypeople, we need put forword a goal of healthy aging. The communitynursing is becoming more important, which based on China’s specialnational conditions family become small and core.It is worth noting thatchronic disease has become increasingly in low level of the elderlyhealth. Community nursing workers needs starting from the needs ofthe elderly,it will become a fast-growing, emerging service field in thecommunity.However, different community nursing have the differentimplementation of scheme.The process is not identical andnormative.So from the point of human nature, providing a appropriatenursing service scheme with the method of specialization andstandardization.The nursing care of traditional Chinese medicine in theelderly health care applications has incomparable advantages.Tocombine traditional Chinese and western medicine in communitynursing work will be the new development of community care. Objective：1.Make a community elderly care plan to community nurses.Make thescheme more especial, standardized, scientific and systematic. It isadvantageous to teach community elderly health in community andschool.2.This study is based on national science and technology plan to buildthe development and application of demonstration in the elderly healthservices platform. Designing the questionnaire about the elderly healthcare needs under the guidance of the framework of the project.3.To improve the quality of life of the elderly to provide the safeguard forthe aged care and the reliable foundation for difficulties situation of theelderly.4.Development of combine traditional Chinese and western medicinenursing community care is an important part of Chinese nursing careerplanning outline.Methods:1.Nursing plan formulation:Based on investigation of Jiangsu provinceof Nantong community and the elderly community of Beijingfor range of158for health care needs in the survey.Choosing thehighest demand of care services, combined with the literatureinvestigation and study, people familiar with the interview results,preliminary set community old people which combine traditionalChinese and western medicine nursing plan(high blood pressure).2.Nursing plan of proving research: for advice of Chinese academy ofsciences, Hubei university of Chinese traditional medicine, College ofnursing,Hubei provincal hospital of TCM and communities workers using the Delphi method. Then make nursing scheme applied to theelderly community in Beijing.Randomly selected10elderly to provefeasibility and rationality of nursing plan.Results:1.Formulate community elderly nursing scheme with traditional Chineseand western medicine nursing plan in hypertension. Including theimplementation of community nursing plan formulation process andcommunity elderly with traditional Chinese and Western medicinenursing care plan implementation process in hypertension.2.In the survey of158elderly showed that four aspects of traditionalChinese medicine nursing in chronic disease care, disease prevention,health promotion,and chronic care needs, especially the daily bloodglucose monitoring accounted for71.5%; Secondly in TCM nursingdemand is relatively accounted for84.4%.3.Designing the community elderly nursing scheme with traditionalChinese and Western medicine nursing in the use of the Delphi method;Using the scheme for the elderly in the community of Beijing, thenanalysis the elderly satisfaction of evaluation sheet.Conclusion:1.This research of community elderly nursing scheme with combinetraditional Chinese and western medicine is practical,standardization;on one hand,developing detailed nursingimplementation flow chart; on the other hand,as the guidance of theelderly demand is suitable for implementation of community servicesenvironment.2． Nursing scheme of combine traditional Chinese and westernmedicine is scientific and add to the nursing service content.It is a guidefor training nursing professionals.It is beneficial to the physical and mental health of the elderly and to applicate in the community care.
