Objiective:the current study aims to find the correlation between the extent of coronarylesions with ankle-brachial index (ABI)and serum homocysteine(Hcy),it also contain apreliminary study of the predictive value of coronary lesions for ABI and exerciseABI.Method:117patients who were suspected as coronary heart disease(CHD) werecollected,all the patients were divided into normal group and CHD group according tocoronary arteriography results, the CHD group were further divided into mild-moderatelesions group and severe lesions group. all the patients were measured with ABI andexercise ABI,and serum Hcy、lipid、glucose were detected in preprandial blood.TheABI、Hcy and Gensini’s Score among groups were compared, the correlation betweenABI、Hcy and coronary lesions were discussed,the ROC curve of ABI’s and exerciseABI’s disgnostic value were compared.Results:1.the difference of ABI between normal group and CHD group was notsignificant, the difference of ABI between mild-moderate lesions group and normalgroup was not significant, the ABI between normal group and severe lesions group hadsignificant difference.The Gensini score and Hcy’s differences among groups weresignificant. We found negative correlation between the resting ABI and coronarylesions,the correlation was-0.790,P<0.01,we also found positive correlation betweenthe Hcy and coronary lesions,the correlation was0.616,P<0.01.2.the difference ofexercise ABI between mild-moderate lesions group and normal group was notsignificant, the exercise ABI between normal group and severe lesions group hadsignificant difference.Compared with the resting ABI, the exercise ABI inmild-moderate lesions group and normal group had no change or a slight rise,and therange was less than20%,all the ABI value were in the normal range.the exercise ABI insevere lesions group mostly decreased than resting ABI,some patients with normal resting ABI had a decreasing exercise ABI,the range were more than20%and ABI<0.9.3.The resting ABI’s AUC to diagnose serious coronary heart disease were0.576, P<0.05,to diagnose serious coronary lesions were0.714, P<0.01,The excecise ABI’sAUC to diagnose serious coronary heart disease were0.629,P<0.05,to diagnoseserious coronary lesions were0.799. P<0.01,We had a preliminary conclusion thatexcecise ABI may have better sensitivity in predictive value ofserious coronary lesions.Conclusion:The resting ABI,Hcy are associated with extent of coronary lesions,excecise ABI may have better predictive value of serious coronary lesions.
