BackgroundsAs the population ages, chronic heart failure has increasingly become an important serious threat to human health in recent years. It is clear that the basic mechanisms of heart failure is ventricular remodeling. Ventricular remodeling characterized by changed cardiac structure and ventricular function clinically. Cystatin C is a sensitive indicator of early renal function damage. Recent studies suggested that cystatin C is an independent risk factor for cardiac death. We also found that serum cystatin C concentration and heart function grade of NYHA in ischemic heart failure patients were positively correlated. With the increase of cystatin C concentration in serum, cardiac mortality rates in patients with heart failure increased significantly. TGF-β1is a multifunctional polypeptide growth factor. Almost all the karyocytes can synthesize and release TGF-β1. It is associated with a variety of biological processes such as cell multiplication, cell differentiation, cellular immunity, inflammation and so on. It has confirmed that TGF-β1can promote the fibrosis and the accumulation of extracellular matrix. As serum markers of cardiovascular disease, cystatin C and TGF-β1had been paid more and more attention in recent years.ObjectiveTo observe the serum levels of cystatin C and TGF-β1in patients with chronic heart failure. And to investigate the relationship between those parameters and cardiac function and ventricular remodeling in patients. Methods96cases of patients with chronic heart failure visited the First Affiliated Hospital of Zhengzhou University from January2010to December2012were included. According to the Classification of function capacity of the NYHA, patients were divided into three groups:22cases of NYHA Ⅱ,42cases of NYHAⅢ,32cases of NYHA Ⅳ, and40patients as control subjects. Serum cystatin C levels were measured by latex enhanced immune turbidimetry, and the levels of TGF-β1were determined by enzyme-linked immunosorbent assay. Echocardiography was employed for quantifying left ventricular end-diastolic diameter(LVEDD), interventricular septum thickness(IVST), left ventricular posterior wall thickness(PWT), ejection fraction(LVEF) and so on. Furthermore, left ventricular mass index was calculated, and make a correlation analysis on cystatin C and TGF-β1.Results1. The levels of cystatin C and TGF-β1,and LVMI in patients with chronic heart failure were significantly higher than those in normal control group(P<0.01);2. Blood levels of cystatin C and TGF-β1and LVMI elevated significantly in chronic heart failure patients with NYHA functional class Ⅱ, Ⅲ and Ⅳ than those in normal control group. With the increase of NYHA heart function classification, the above indicators increased too(P<0.01);3. Linear correlation analysis showed that cystatin C and TGF-β1are positively correlated with LVMI.Conclusion1. Elevated levels of cystatin C and TGF-β1might be a predictor of heart failure;2. The levels of cystatin C and TGF-β1elevated with the increase of NYHA classification;3. Cystatin C and TGF-β1concentration were positively correlated with left ventricular mass index, which can be used as evaluation index of ventricular remodeling in chronic heart failure. And both of them may be helpful for the early diagnosis of chronic heart failure.
