Objective:The aim of this study is to investigate the change of serum cystatin Cconcentration in patients with chronic heart failure and explore the relationships ofcystatin C and left ventricular remodeling in chronic heart failure.Methods: chose120patients who were hospitalized in the geriatric departtment of theYi Ji Shan Hospital from March2013to Dec2013,which were diagonosed as theChronic heart failure(67males,53females, aged52-81years, mean65.3±9.6yearsold)and50cases of normal control group(49males,36females, aged46-76years, mean60.9±8.9years old),all the groups exclude serious liver and kidney dysfunction (uremia,etc).According to New York Heart Association functional class we divide this heartfailure patients into three subgroups (below Ⅱ:44cases, Ⅲ:57cases, Ⅳ19cases).peripheral blood cystatin C,left ventricular end-diastolic volume (LVEDV), leftventricular ejection fraction (LVEF) were measured in heart failure group.UsingDevereux ventricular weight formula to calculate left ventricular mass index(LVMI).Statistical analysis:SPSS17.0statistical package for data analysis, test all thedata whether they are normally distributed using One Sample Kolmogorov-SmirnovTests,mark the measurement data with mean±standard deviation (x±s) in all observedindicators,mark the Count data with chi-square test,P <0.05was statistical difference.Results:serum cystatin C concentrations in chronic heart failure group (1.395±0.35mg/L) was significantly higher than the normal heart function group (0.952±0.26mg/L)(P <0.05), cystatin C concentrations are significantly increased in NYHA Class Ⅲ andNYHA Class Ⅳwith respect to subgroup of NYHA Class I,II(P<0.05).The difference of serum Cys-C concentrations between NYHA Class Ⅲ and NYHA Class Ⅳgroupwas not statistically significant (P>0.05).serum Cys-C concentrations was positivelycorrelated with IVST,PWT,LVDd,LVMI. r(IVST)=0.562. r(LVDd)=0.516, r(LVPW)=0.359.r(LVMI)=0.645, P<0.05,and negatively correlated with left ventricular ejection fraction (r=-0.546P <0.05).Conclusion:Serum cystatin C in patients with chronic heart failure were significantlyhigher than those without clinical manifestations patients, and cystatin C concentrationwas positively correlated with ventricular remodeling, and cystatin C can be used asevaluation index of ventricular remodeling in chronic heart failure.
