Objective: Thyroid carcinoma is a common malignant tumor in women,with differentiated thyroid cancer is most common, thyroid cancer incidence isnearly20years the fastest growing entity malignant tumors, with an averageannual growth rate of6.2%. Now it is common tumor and fifth for malignanttumor of female. Growth and cell proliferation and apoptosis of infinitemalignant tumor inhibition of estrogen and progesterone are closely related,through action and its receptor ER, PR could stimulate cell proliferation, andthe newly discovered inhibitor of apoptosis protein Survivin, expressed in avariety of malignant tumors, which can inhibit cell apoptosis. Effects ofthyroid cancer by estrogen, and is affected by apoptosis inhibiting geneSurvivin so far inconclusive. The detection of ER, PR and Survivin proteinexpression in thyroid carcinoma of two in thyroid cancer development, roleand relationship of transfer, provide a new theoretical basis for the clinicaldiagnosis, treatment and prognosis.Method:1Select the Third Hospital of Hebei Medical University resectionoperation pathology archive, complete clinicopathological data between2003-2012thyroid tissue, all specimens were obtained after further review ofpathology physician. All cases were as the first, had not received preoperativeradiotherapy and chemotherapy. Select typical paraffin-embedded tissueblocks, randomly selected20cases of papillary carcinoma;20cases offollicular carcinoma;20cases of nodular goiter;20cases of thyroid adenoma.2Using immunohistochemical method, detected the expression of ER,PR and Survivin in differentiated thyroid carcinoma, nodular goiter, thyroidtumor tissues.3Using SPSS17.0statistical analysis software, data using the variance of multiple samples were analyzed, test of homogeneity of variance test, t testand Pearson correlation, test level for the P=0.05.Results:1The results of ER immunohistochemical expression:ER was expressed in the thyroid papillary carcinoma, follicularcarcinoma, thyroid adenoma and nodular goiter tissue, volume fraction(IS)were3.90±3.32,3.65±2.73,2.10±1.44and1.90±1.59. The expressionlevel of ER in papillary thyroid carcinoma tissues was significantly higherthan that of nodular goiter and thyroid adenoma tissues (P<0.05), comparedwith the expression in follicular carcinoma tissues, the difference was notstatistically significant (P>0.05). The expression of ER in follicular thyroidcarcinoma was significantly higher than thyroid adenoma and nodular thyroidtissue (P<0.05). Expression of ER in thyroid adenoma and nodular thyroidtissue, the difference was not statistically significant (P>0.05). Whether inpapillary carcinoma and follicular carcinoma, and no obvious correlationbetween ER expression and the age, gender (P>0.05), but ER in papillarycarcinoma lymph node metastasis in papillary carcinoma was significantlylower than that without lymph node metastasis (P<0.05); the expression of ERin follicular carcinoma, metastasis no significant correlation with lymph node(P>0.05).2The results of PR immunohistochemical expression:PR was expressed in the thyroid papillary carcinoma, follicularcarcinoma, nodular goiter and thyroid adenoma tissues, volume fraction(IS)were3.15±2.62,3.75±2.83,2.05±1.47and2.10±1.65. Expression levelsin thyroid papillary carcinoma and expression of PR in follicular carcinoma,nodular goiter and thyroid adenoma tissue showed no significant difference(P>0.05), the expression of PR in follicular carcinoma tissues wassignificantly higher than that of nodular goiter and thyroid adenoma (P<0.05).Expression of PR in thyroid adenoma and nodular thyroid tissue, thedifference was not statistically significant (P>0.05). Whether in papillarycarcinoma and follicular carcinoma, and no obvious correlation between PR expression and the age, gender (P>0.05), the expression of PR in papillarycarcinoma lymph node metastasis in papillary carcinoma was significantlylower than that without lymph node metastasis (P<0.05); the expression of PRin follicular carcinoma, no metastasis was significantly associated with lymphnode (P>0.05).3The results of Survivin immunohistochemical expression:Survivin was expressed in the thyroid papillary carcinoma, follicularcarcinoma, thyroid adenoma and nodular goiter tissue. Volume fraction(IS)were2.55±1.50,2.20±1.61,1.15±0.93and1.20±0.89. The level ofexpression of Survivin in papillary thyroid carcinoma was significantly higherthan that of nodular goiter and thyroid adenoma tissues (P<0.05), comparedwith the expression in follicular carcinoma tissues, the difference was notstatistically significant (P>0.05). The expression of Survivin in follicularthyroid carcinoma was significantly higher than thyroid adenoma and nodularthyroid tissue (P<0.05). No significant difference of Survivin expression inthyroid adenoma and nodular goiter tissue (P>0.05). Whether in papillarycarcinoma and follicular carcinoma, and no obvious correlation betweenSurvivin expression and the age, gender (P>0.05), associated with lymph nodemetastasis, the expression of Survivin in papillary carcinoma and follicularcarcinoma lymph node metastasis was significantly higher than that withoutlymph metastasis of papillary carcinoma and follicular carcinoma (P<0.05).4Correlation between expression of ER, PR, Survivin in differentiated thyroidcarcinoma:Further analysis showed that, ER was positively associated with PRexpression in papillary thyroid carcinoma, and ER and Survivin, PR andSurvivin expression in which thyroid papillary carcinomas were negativecorrelation (P<0.05); ER and PR positive expression correlation in follicularcarcinoma (P<0.05), and ER with Survivin, PR and Survivin expression infollicular carcinoma and had no significant correlation (P>0.05).Conclusions:1Expression of ER, Survivin in thyroid papillary carcinoma were significantly higher than that of nodular goiter and thyroid adenoma, and inthe PR expression had no significant difference between the three; theexpression of ER, PR and Survivin in thyroid follicular carcinoma tissues wassignificantly higher than that of thyroid adenoma and nodular thyroid goiter.Compared with papillary thyroid carcinoma and follicular thyroid carcinoma,ER, PR, Survivin expression had no significant difference.2Expression of ER, PR, Survivin in thyroid papillary carcinoma wereassociated with gender, age not related, are associated with lymph nodemetastasis in lymph node metastasis, decrease ER and PR expression inpapillary thyroid carcinoma, but Survivin protein expression increased; andER and PR in papillary thyroid carcinoma with expression positive correlationwith Survivin, ER, PR and Survivin expression in carcinoma of thyroidpapillary carcinoma has a negative correlation; expression of ER, PR inthyroid follicular carcinoma in the gender, age and are not related and lymphnode metastasis, and Survivin protein expression and lymph node metastasisin lymph node metastasis, high expression of thyroid follicular cancer tissues.ER and PR expression in thyroid follicular carcinoma tissues had positivecorrelation with Survivin, ER, PR and Survivin in carcinoma of thyroidfollicular carcinoma showed no significant correlation.3ER, PR and Survivn may play a certain role in the occurrence anddevelopment of differentiated thyroid carcinoma and lymph node metastasisprocess.
