Objective:To observe the therapeutic effect of Yi Qi Tong Mai decoction on Prethrombotic Sate (PTS) improvement and Deep Vein Thrombosis (DVT) prevention in senile patients received major hip surgery.Methods:16patients (male8, female8, average age76.9) received major hip surgery (Total hip arthroplasty5, Artificial femoral head replacement2, Proximal Femoral Nail Anti-rotation surgery9) were selected in Orthopedics Department of Guang An Men hospital, China Academy of Chinese Medical Sciences (CACMS) from October2011to March2012. They were randomly drew lots to divide into two groups (Experimental group and Control group). The treatment group was given Yi Qi Tong Mai decoction per oral, The control group was given Low Molecular Weight Heparin (LMWH) hypodermic injection, as the anticoagulant therapy. The observation period was2weeks, the patients’ clinical symptoms, physical signs, bleeding tendency, and the changes of color Doppler ultrasound were documented. And compared the variation of hemorheology, Blood cells Analysis, CRP, ESR, prothrombin time (PT), activated partial thromboplatin time (APTT), D-Dimmer, Fibrinogen, total cholesterol (TC), triglyceride (TG) and the DVT rate, at the time of pre-operation,1week post-operation and2weeks post-operation, between the two groups.Results:1. The APTT of two groups was prolonged compared to preoperative level at1week and2weeks pos-operation (P<0.05), the PT of control group was longer than preoperative level at2weeks pos-operation (P<0.05). The D-Dimmer level fo two groups reduced at1week and2weeks pos-operation (P<0.05). The PT level of control group was longer than experimental group at2weeks pos-operation (P<0.05), other items of DIC screen had no significant difference between two groups (P>0.05).2. The RBC aggregation index (RAI), RBC deformability index (RDI), and the TC level of experimental group were reduced than preoperative level, at2weeks pos-operation (P<0.05). The reduction viscosity of blood (Low cut) level of control group was reduced than preoperative level, other items of hemorheology had no significant difference between pre and post operation in two groups (P>0.05). The reduction viscosity of blood (High cut) level of experimental group was higher than control group (P<0.05), the RAI of experimental group was lower than control group (P<0.05), other items of hemorheology had no significant difference between two groups2weeks pos-operation (P>0.05).3. The PLT counts raised in two groups pos-operation (P<0.05), and the RBC counts, HCT level descended pos-operation (P<0.05), other other items of Blood cells Analysis, had no significant difference between pre and post operation in two groups (P>0.05). The ESR level of experimental group was lower than control group at2weeks after surgery (P<0.05).4. The postoperative hemorrhage amount of experimental group was less than control group (P<0.05). The DVT rate and the complications rate of two groups had had no significant difference (P>0.05).Conclusion:The Yi Qi Tong Mai decoction had the efficacy of anticoagulant and thrombolytic, it can improve the Prethrombotic Sate, and prevent Deep Vein Thrombosis of senile patients received major hip surgery, by reducing the inflammatory cytokines in the blood, reducing total cholesterol level, descending the erythrocyte aggregation level and improving its deformability. Meanwhile it did no increase the risk of bleeding. The LMWH had better efficacy of improving blood viscosity. As the limited of less cases number and short observation period, further efficacy observation need to be improved.
