| Objective:To find out the regulation and idea of Chinese medicine in treating pulmonary interstitial fibrosis, and to provide new clinical basis for Chinese medicine in treating pulmonary interstitial fibrosis by means of study on the Chinese herbal compound for treatment of pulmonary interstitial fibrosis.Methods:Retrospective study is employed in this study. Cases that meet the inclusion criteria are selected from the inpatient medical records of our department.182prescriptions are selected from the cases and input into a Microsoft Excel file. They are sorted by names, natures and flavors. Then, the input data are cleaned as follows:firstly, the names are normalized and the combined concepts are separated; meanwhile, digital processing is conducted, i.e. to assign values to natures in accordance with the method of Jiang Shirong’s "Solving All Multipliers of a Number". The values consistent with some description are marked with "1" and those inconsistent with some description are marked with "0". The frequency analysis method is employed to calculate the frequency of usage and the types of flavors. The hierarchical cluster analysis is conducted for medicines whose frequency is over18. The idea of the medicine in treating pulmonary interstitial fibrosis is analyzed through medicine classification. The correlation rule analysis is also employed. The interested couplet medicines and medicine groups are obtained through setting the appropriate minimum support, minimum confidence and correlation. Then, the combination rules are summarized through analysis of the couplet medicines and medicine groups.Results:1. In the182compounds,206medicines are used with a total dosing frequency of2480, among which, there are43kinds of medicines that are used more than18times with a total dosing frequency of1669.2. The43kinds of common medicines are divided into eight categories through cluster analysis, which are reinforcing medicine, clearing heat and eliminating phlegm medicine, medicine regulating activities of qi, aperients, soothing liver and activating blood circulation medicine, easing nature medicine, astringent and meridians-relieving medicine respectively. The main therapeutic effects of reinforcing medicine are invigorating qi, strengthening spleen and regulating stomach; the main therapeutic effects of clearing heat and eliminating phlegm medicine are clearing lung, eliminating phlegm and nourishing yin; the main therapeutic effects of medicine regulating activities of qi are ventilating lung, bringing adverse qi downward and promoting the circulation of qi; the main therapeutic effects of aperients arc eliminating phlegm, activating blood circulation and moistening the intestines; the main therapeutic effects of soothing liver and activating blood circulation medicine are soothing liver and activating blood circulation; the main therapeutic effects of easing nature medicine are strengthening the middle warmer, detoxifying and easing the nature; the main therapeutic effects of astringent are astringing lung and nourishing kidney; the main therapeutic effects of meridians-relieving medicine are dredging collaterals and relieving asthma.3. Through correlation rule analysis,41interested correlation rules of couplet medicines and31pairs of couplet medicines are obtained;60interested correlation rules of medicine groups and24medicine groups are obtained.Conclusions:1. Through study on the8categories of medicines obtained by cluster analysis, it is concluded that:the therapies of invigorating qi, eliminating phlegm and activating blood circulation are employed based upon the main pathogenesis of pulmonary interstitial fibrosis as weakened body resistance and collaterals blocked by phlegm-stasis. The spleen and stomach of a patient is recuperated by taking strengthening spleen and regulating stomach medicine, to improve his ability to convert the essence of water and grain and to "reinforce earth (spleen) to generate metal (lung)". The astringing lung and nourishing yin medicine is also used in consideration that the weakened bodv resistance has last for a long time and the deficiency of lung qi has developed as deficiency of both lung qi and kidney yin. It is also found that resistance in veins not only results in phlegm stagnation, but causes the issue of qi stagnation. Besides the application of soothing the liver, promoting the circulation of qi, ventilating lung and depressing qi, the exterior and interior relationship of zang-fu viscera shall be aiso be taken into consideration. For example, lung qi being unable to descend can be caused by constipation, which is one of the mechanisms of aperients; the mechanism of the meridians-relieving insect medicine is to directly function at the blood system and to remove the blood stasis based upon the character of static blood in obstructed collaterals.2. Through the analysis of couplet medicines and medicine groups, it is found that the prescription-formulating principles of couplet medicines are to choose medicines with similar functions, to combine classic medicines, to use fixed couplet medicines, or to supportively treat the side reaction of couplet medicines with therapeutic action. It is also found that the prescription-formulating principles of medicine groups are to use the basic herbs of classic prescriptions, to combine herbs of several classic prescriptions, and to change the herbs of the classic prescriptions, or to use one couplet medicines as the basic medicines for metaplasia of other medicine groups. Mutual confidence of these medicine groups are100%, which indicates that they can only be used as couplet medicines.3. To sum up, the data mining tools are useful for studying regulation and clinical value of Chinese herbal compounds in treating pulmonary interstitial fibrosis. |
