Study Of PFMEA Application And Improvement At SHB Company

In the last decade, a big number of products were found with quality problems with sizable consequences. Some even caused close down, or severe infringement to customers" interests, meanwhile its own brands images were substantially damaged as well. Quality management is essential to survival and development of a company. The root causes of most quality failures lies in failures control during production process. Failure control starts with failure analysis, and then achieves the improvement by field process control. Therefore, failure analysis and Held failure management is critical to quality control.This research described SHB’s situation of quality risk, studied application of the methodology "Process Failure Modes and Effects Analysis"(PFMHA). also investigated their practice of this methodology, and found the inaccuracy of identifying quality failure and ineffectiveness of control on failure causes. The deeper study identified the root causes of identification inaccuracy are lack of involvement of key person on production process, and lack of proper requirement to mass production in work flow document, and improperness of rating criteria. Discovered by further investigation on ineffectiveness of control on failure causes, major causes are identified quality failures are not listed in control documents of production process, and then therefore the control doesn’t function.Based on the above investigation and finding, the problem of inaccuracy of identifying quality failures is solved by enhance team portfolio, complementing work process for mass production, and improving rating criteria; another problem of ineffectiveness of control on failure causes is solved with establishing connection between identified failures and process control documents for production. The above approaches are all tested and verified in practice.
