The Clinical Application Of Neutrophil-to-lymphocyte Ratio And It Combines With CA125in Endometriosis

Objective: Study of the clinical application of neutrophil-to-lymphocyte ratio(NLR)and it combines with CA-125in endometriosisMethods: Perform a retrospective review of endometriosis, benign ovarian tumors,health check people,in the first hospital affiliated of Dalian Medical University fromSeptember2008to December2011.59cases had pathologically confirmedendometriosis at the time of surgery. Among those cases27were stage I and II,32werestage III and IV according to the American Fertility Society (AFS).46cases of benignovarian tumors, physical examination revealed no abnormalities41cases as controlgroup by strict screening standards. Sensitivities and specificities of serum CA-125,NLR and combined markers (CA-125*NLR) for diagnosis of endometriosis wereevaluated by receiver-operating characteristic (ROC) analysis. And analyze thecorrelations of combined markers and the severity of endometriosis.Results: The level of NLR and combined markers in endometriosis wassignificantly higher than the other two groups (P<0.05). The level of CA-125inendometriosis was also significantly higher than the other two groups (P<0.05), And instage I and II is lower than stage III and IV. the difference was statistically significant(P<0.05).Sensitivity of NLR, CA-125and combined markers were(72.9%,49.7%,83.3%),respectively and specificitiy were (60.7%,94.5%,72.4%).Respectively at the cutoff value. The sensitivity of NLR and combined markers ishigher than CA-125, however CA125is higher than NLR and CA-125in the specificity.The positive rate of NLR and combined markers was higher than CA-125, in stage I andII endometriosis group.the difference was statistically significant (P<0.05).and thepositive rate of combined markers was also higher than CA-125, in stage III and IVendometriosis group. the difference was statistically significant (P<0.05). The positiverate of CA-125in stage III and IV was higher than in stage I and II endometriosis group, the difference was statistically significant (P<0.05).Conducting relative analysis ofNLR，CA-125，combined markers and the severity of endometriosis，no obviouscorrelation between NLR and stages of the disease were observed(r=0.125, P=0.34)，butthere was a moderate correlation between CA-125, combined markers and stages of thedisease respectively (r=0.661, P<0.001),(r=0.634, P<0.001).Conclusion:The sensitivity of NLR maybe better than CA-125in screening forendometriosis.and combined with CA-125may lead to a more accurate diagnosis ofendometriosis.
