BackgroundHyperplasia of mammary gland is one of the Benign Proliferative Breast Disease (BPBD). Although BPBD has been extensively studied, the aetiology of this disease is still poorly understood. It appears that endocrine factors are associated with Benign Breast Disease, however, there are also other inconsistent results like obesity, smoking, exogenous hormone use, caffeine intake and alcohol are possibly associated with BBD.The most extensive prevention therapy to BBD in Western Medicine is chemoprevention agents such as Tamoxifen, Raloxifene, Aromatase Inhibitors and so on. But some BBD cases which have relative risk on developing future breast cancers are suggested to be treated by surgical therapy. It is known that chemoprevention agents have different kinds of side effects, so this therapy is not well-accepted by most patients.BBD is considered as RUPI in Traditional Chinese Medicine. There have been many useful information in Chinese old literature, regarding Qi-Blood imbalance, disorder of Liver Qi and lack of Kidney function as the most important causes of RUPI. Older TCM scholars found and developed many kinds of methods to treat this disease, like regulate Qi-Blood system, coordinate Liver Qi and Spleen-Stomach function, enhancing Kidney Qi and Yin Yang etc. Even though we are still lack of literature about acupuncture therapy of BBD. this study absorbed older treatment experiences and accepted Doctor Wang’s guideline to make a unique acupuncture therapy—electro-acupuncture and blood-letting therapy. Comparing with chemoprevention agents or surgical therapy, acupuncture is safer, more well-accepted and cheaper.This paper includes three parts, the first part is a literature review involved ancient and modern study about Benign Breast Disease. Part two is about this clinical research, including materials, methods and results. We analyze the data and discuss the results in part three, explaining the roles of acupoints used in this research.Objective:To research the clinical effect of hyperplasia of mammary gland by electro-acupuncture and blood-letting; to explain how acupuncture affected on treating hyperplasia of mammary gland.Methods Design:Prospective cohort study, parallel trial, blank controlled. Patients:36patients had hyperplasia of mammary gland, selected from Acupuncture Department of Guang’an men Hospital, are divided into two groups.Interventions:One was blank control group which given no-medical treatment and the other one was acupuncture treatment control group given electro-acupuncture and blood-letting.Measurements:Record the symptom score of both groups before and after one complete treatment course. Symptom score contained breast pain score, breast tumor score, other relative symptom score.Primary outcomes were breast pain and breast tumor. The two groups’score of breast pain and breast tumor was measured, while effective rate of two different measurements was compared after one complete treatment course. We use SPSS19.0statistical software to analyze all data appears in this subject.Course and Frequency:The whole study was5months to8months, which contained2months of treatment period and3months to6months of follow-up period. Patients in acupuncture group were treated with EA twice a week for12times and had a break during menstrual period. Patients in blank control group were treated non-medical treatment except psychological counseling.Result:(1) Breast Pain-relief:After one complete treatment course, compared two groups’ symptom score from breast pain. Before and after one course, the acupuncture treatment group’s breast pain score were23.18±3.36and9.55±5.75, and there was a statistically significant difference between them(p<0.05).(2) Breast tumor decrease:Before and after one course, the acupuncture treatment group’s breast tumor score were11.93±5.58and7.16±5.65. There was a statistically significant difference between them (p<0.05), while there was no statistical difference between before and after one observation in blank control group (p>0.05).(3) Effective rate:We used two different measurements in this study, there was subtle difference between them, but the total effective rate is all86.36%, and there was no statistical difference (p>0.05).Conclusions(1) Electro-acupuncture could be a effective treatment to hyperplasia of mammary gland for it could decrease not only breast pain and tumor but improve other relative symptoms.(2) We could see the negative results of blank control group, the spontaneous cure in hyperplasia of mammary gland was not so good. It would be better to be treated by medical intervention for this kind of patients.
