Objectives:The present work was to detect the-344C/T polymorphism of aldosteronesynthase(CYP11B2) gene, and the concentration of the plasma endothelin-1(ET-1)and nitric oxide (NO) in essential hypertension patients with obesity. To study thesusceptible genotype and allele gene for essential hypertension patients with obesity,and the relationship between the-344C/T polymorphism of CYP11B2gene and theconcentration of the plasma ET-1/NO.Methods:60cases of1to2grade essential hypertension patiens from Department ofCardiovascular Medicine, and60people from the Examination Center of SecondAffiliated Hospital of University of South China during2011–2012were enrolled.All the people were between20to60years old. Blood pressure and body weight weremeasured in the early morning fasting state. Enrolled patients samples were collectedby the diagnostic criteria for essential hypertension of "Guidelines for the Preventionand Treatment of Hypertension in China2010" and the diagnostic criteria for obesityof "Guidelines for Prevention and Control of Overweight and Obesity in ChineseAldult", and excluding related diseases according to exclusion criteria. All subjectswere divided into four groups:30cases essential hypertension with obesity group(group A),30cases essential hypertension group (group B),30cases obesity group(group C),30cases healthy control group (group D). the-344C/T polymorphism ofaldosterone synthase gene were detected using the polymerase chain reaction(PCR),restriction fragment length polymorphism(RFLP), agarose gelelectrophoresis method andother methods. ET-1concentration was measured by enzyme linked immunosorbentassay(ELISA), serum NO was examined by the method of nitrate reductase. Statistieal package of SPSS13.0was applied for the statistical analysis.Results:1.TT, TC, CC genotypes Distribution of each group: essential Hypertension withobesity group were24(80%),4(13%),2(7%); essential Hypertension group were21(70%),6(20%),3(10%); obesity group were22(73%),6(20%),2(7%); healthy controlgroup were12(40%),10(33%),8(27%). The comparison of genotype distribution foreach group showed there were significantly different (x2value were12.559, P<0.05).Multiple Range Test in TT genotype the number of cases and TC+CC genotypes thenumber of cases for each group showed there were significant difference as essentialhypertension with obesity group, essential Hypertension group and obesity groupseparately compared with healthy control group (x2value separately were10.000,5.455,6.787, P<0.05), essential Hypertension with obesity group was in the highestsignificant difference.2.T and C alleles gene frequency Distribution of each group: essential Hypertensionwith obesity group were respectively52(87%) and8(13%); essential Hypertensiongroup were respectively48(80%) and12(20%); obesity group were respectively50(83%) and10(17%); healthy control group were respectively34(57%) and26(43%).The comparison of distribution alleles gene showed there were significant differencein each group (x2value were18.634, P<0.05). Multiple Range Test in T and C allelesgene frequency the number of cases for each group showed there were significantdifference as essential hypertension with obesity group, essential Hypertension groupand obesity group separately compared with healthy control group (x2value separatelywere13.297,7.548,10.159, P<0.05), essential Hypertension with obesity group wasin the highest significant difference.3.The comparison of the plasma endothelin-1and nitric oxide concentration of eachgroup, results showed: essential Hypertension with obesity group, essentialHypertension group and obesity group compared respectively with healthy controlgroup showed significant difference (P<0.05); essential Hypertension with obesitygroup compared with essential Hypertension group and obesity group respectively showed significant difference (P<0.05); essential Hypertension group compared withobesity group had no significant difference (P>0.05).4.Between all groups, ET-1concentration had significant difference (F value were181.585, P<0.05). The difference of ET-1in TT, TC and CC genotypes weresignificant (F value were4.420, P<0.05). Both each group and genotype hadinteraction, and TT genotype of essential Hypertension with obesity group was in thehighest concentration of ET-1(F value were3.226, P<0.05).5.The values of NO between all groups had significant difference (F valuewere85.873, P<0.05). Meantime, the concentration of NO had significant differencesfor TT, TC and CC genotypes (F value were6.954, P<0.05). Both each group andgenotype had interaction, but TT genotype of essential hypertension with obesitygroup was in the Lowest concentration of NO (F value were8.229, P<0.05).Conclusions:1.The-344C/T polymorphism of CYP11B2gene is associated the development ofessential hypertension patients with obesity, and TT genotype could be susceptiblegenotype.2.The plasma endothelin-1concentration level to increase and the nitric oxideconcentration to decrease in Essential hypertension patients with obesity shows thatthere is a certain dysfunction in vascular endothelium.3.The plasma endothelin-1concentration level to increase and the nitric oxide conce-ntration to decrease in TT genotype of the-344C/T polymorphism of CYP11B2genein essential hypertension patients with obesity indicates that there is a certaindysfunction in the vascular endothelium for TT genotype.
