ObjectiveThe research was designed to investigate the susceptible genotypeand allele, as well as the relationship between aldosterone synthasegene polymorphism and adipose tissue metabolism in essential hyperten-sion patients with obesity by detecting aldosterone synthase(CYP11B2)gene-344C/T polymorphism and adipokines Apelin concentration.MethodsThere were selected for the study at Cardiology of Second Affiliated Hospitalof South China University between2012to2013, included80cases of grade1-2essential hypertension (Essential Hypertension, EH) patients, and80cases of healthysubjects for the study. These patients were in accordance with the "ChineseHypertension Prevention Guide2010" diagnostic criteria of hypertension and the"Chinese adults are overweight and obesity prevention and control guidelines2004"to determine the diagnostic criteria of obesity, and related diseases outside theexclusion criteria. Research will be selected160cases,which were divided into Ⅰ, Ⅱ,Ⅲ and Ⅲfour groups of40patients with essential hypertension were obese,hypertension in non-obese group, simple obesity group and the healthy control group,during the use of the polymerase chain reaction (Polymerase Chain Reaction, PCR)and restriction fragment length polymorphism (Restriction Fagment LengthPolymorphism, RFLP) to detect CYP11B2-344C/T polymorphism;Serum Apelinlevels was measured by enzyme linked immunosorbent assay (ELISA), Finally SPSS18.0software was selected to collect the data of statistical analysis.Results1. Comparing each group general foundation of clinical data, the four sets of datawere comparable:(1)Each experimental group showed no significant difference on gender, age,andFasting Blood Glucose (P>0.05).(2)Body Mass Index (BMI),Waist-to-Hip Ratio (WHR), Triglycerides (TG),TotalCholesterol (TC), Low-Density Lipoprotein cholesterol (LDL) and serumHigh-Density Lipoprotein (HDL) of Ⅰgroup and Ⅲ group, compared with Ⅱgroup and Ⅳ group, were significantly different(P<0.05).(3)The Diastolic Blood Pressure (DBP) and Systolic Blood Pressure (SBP) of Ⅰgroup and Ⅱgroup, compared with Ⅲ group and Ⅳgroup,were significantlydifferent (P<0.05).2. Compare CYP11B2-344C/T genotype and allele frequency distribution(1)The comparison of CYP11B2-344C/T genotype frequency distributionⅠ, Ⅱ and Ⅲ respectively with TT genotype number of cases Ⅳ group with TC+CC type the number of cases for multiple comparisons, there is a significantdifference (x2values were11.16,5.12,4.11, P <0.05), which Ⅰ group differencesmost significant; but Ⅰ and Ⅱgroup, Ⅰ and Ⅲ group, Ⅱ and group Ⅲwere compared, TT genotype proportions were no significant difference (x2valueswere0.58,1,0.058, P>0.05).(2) The comparison of CYP11B2-344C/T allele frequency distributionⅠ, Ⅱ and Ⅲ T and C, respectively, with the number of cases Ⅳ group alleles formultiple comparisons, there is a significant difference (x2values were11.84,5.09,3.56, P <0.05), of which the most obvious difference between group Ⅰ.But Ⅰand Ⅱgroup, Ⅰ and Ⅲ group, Ⅱ and group Ⅲ were compared,the differencewas not statistically significant (x2values were1.56,2.63,0.14, P>0.05).3. The comparison of Apelin concentration between groupsⅠ, Ⅱ, Ⅲ and Ⅳ respectively comparison group, the difference was significant (P <0.05); Respectively, compared with group Ⅰ and Ⅱ group, there was asignificant difference (P <0.05); There was a significant difference (P<0.05) betweenthem compare group Ⅱ and group Ⅲ.4.Relationship between CYP11B2-344C/T genotype and Apelin levels.In each group, Apelin concentration had significant difference (P<0.001). By TT,TC and CC genotype distribution group Apelin concentrations were significantlydifferent between the Ⅰ, Ⅱ, Ⅲ group (P<0.05). The difference of Apelin inⅣ guoup genotypes was no significant (P<0.05). Both each group and genotype hadinteraction, and TT genotype of Ⅰ group was in the lowest concentration of Apelin(P>0.05). Statistical analysis grouped by genotype Apelin serum concentration levelsof each group, TT genotype was significantly lower than TC, CC genotype group (P<0.05); TC genotype groups with the CC genotype group, no significant difference(P>0.05).Conclusions1. The CYP11B2-344C/T polymorphism might be related with the pathogenesis ofessential hypertension patients with obesity, and TT genotype may be susceptiblegenotype.2. The serum Apelin concentration in essential hypertension patients with obesitywere lower, and it showed that there is dysfunction in Adipose tissue metabolism.3. The serum Apelin concentration in TT genotype of The CYP11B2gene-344C/Tpolymorphism were lower, and it showed that TT genotype and the dysfunctionin Adipose tissue metabolism may promote the development of essential hypertension...
