| BackgroundProfessor Deng Chengshan is the nationalwide famous doctor of traditional Chinese medicine (TCM). He has engaged in medical work for more than fifty years. Famous doctor Deng has accumulated rich clinical experience in the treatment of hematologic diseases, and has formed his own unique academic thoughts. Chief physician Hu Xiaomei is the academic heir of Deng Chengshan."Outline of innovation and development of Chinese medicine (2006-2020)", which was promulgated by the State Council, pointed out that "equal emphasis on inheritance and innovation is one of the basic principles of developing TCM". Then, inheritance of academic experience of famous TCM doctors is important and significant for developing TCM.Idiopathic thrombocytopenic purpura (ITP) is a common hemorrhagic disease caused by peripheral platelet destruction. Treatments of ITP in the Western medicine are with hormone, large dose of immunoglobulin, splenectomy and immune inhibitors combined with chemotherapy. However, there are still30%-50%of ITP with repeated attacks or converted into refractory, and side effects of the Western medicine. This kind of disease is named as "purpura" in TCM, and TCM is superior to treating purpura (ITP). I had the honor of following famous TCM doctor Deng Chengshan and my advisor Hu Xiaomei in clinical. In the present study, inheritance and development of academic thoughts between famous doctor Deng and my advisor Hu were investigated by analizing the academic origin, characteristics and clinical experience of two teachers in treating purpura (ITP).ObjectiveTo investigate and analize inheritance and development of academic thoughts between famous doctor Deng Chengshan and chief physician Hu Xiaomei in the treatment of purpura (ITP).MethodsIn my clinical of following famous doctor Deng Chengshan and chief physician Hu Xiaomei, inheritage of academic thoughts between doctor Deng and director Hu was analized by combing the academic origin, academic point of view as well as academic specialties of the two teachers in treating purpura (ITP). Application of academic thoughts between famous doctor Deng and teacher Hu was analized by collecting the clinical cases and by comparing the multi-factors of theory, methods, formulas and medicinals in patients with purpura (ITP). Development of academic thoughts between famous doctor Deng and teacher Hu was explored by analizing the similarities and differences of academic point of view and clinical application of the two teachers in treating purpura (ITP).Results1. Analysis of inheritage of academic thoughts between famous doctor Deng and teacher Hu in treating purpura (ITP)1.1Academic origins of two teachers Professor Deng Chengshan inherited academic thoughts of famous TCM doctors Wang Wending and Zhu Yan. At the same time, Professor Deng studied hard the Four Great Classics of TCM and the other ancient books of TCM, and gradually formed the academic thoughts focused on tonify deficiency, clearing heat and promoting circulation and removing stasis in the treatment of hematologic diseases.Chief physician Hu Xiaomei inherited academic thoughts of the famous TCM doctors Zhou Aixiang and Deng Chengshan, She also attached great importance to the ancient literatures of TCM, particularly praised highly the Supporting Yang School, which established the academic thoughts focused on Warming Yang of teacher Hu in the treatment of hematologic diseases.1.2Academic viewpoints of two teachersFamous doctor Deng believe that purpura (ITP) is a syndrome of asthenia in origin and asthenia in superficiality. The pathogenic factors of purpura (ITP) include deficiency, heat and blood stasis. The disease location is the kidney. He think that syndrome of deficiency is caused by the deficiency of Qi, Yin and blood and syndrome of blood stasis is caused by the blood circulating out of vessels. Teacher Hu believe that the pathogenic factors of purpura (ITP) include deficiency and blood stasis. The disease location is the kidney. She think that syndrome of deficiency is caused by the deficiency of Qi or Yang and syndrome of blood stasis is caused by bone marrow stasis.1.3Academic characteristics of two teachersFamous doctor Deng treated the patients by tonify deficience, clearing heat, and removing blood stasis, which was the basic law of therapy in inpurpura (ITP). Deficience would be tonified by supplementing qi and nourishing yin and blood. Heat could be cleared by removing pathogenic heat from blood, blood stasis could be dissolved by promoting circulation and removing stasis. Famous doctor Deng emphasis on differentiation combined with disease. He used to have the basic prescription of combination of ErZhi Pill with DangGui BuXue Decoction for tonifing deficience and XiJiao DiHuang Decoction for clearing heat. Chief physician Hu Xiaomei treated the patients by warming Yang and activating Qi, which was the basic law of therapy in inpurpura (ITP). Based on the treatment of Warming Yang and Activating Qi, heat could be cleared by removing pathogenic heat from blood, and blood stasis could be dissolved by promoting circulation and removing stasis. She used to have the basic prescription of combination of Golden Cabinet’s Kidney Qi Pill with SiJunZi Decoction.2. Analysis of application of academic thoughts between famous doctor Deng and teacher Hu in treating purpura (ITP) by comparing the multi-factors2.1Frequency analysis of herbsFrequency analysis of herbs used by two teachers showed that Selaginella Involvens occupied the first, which indicated that famous doctor Deng and teacher Hu were fond of Selaginella Involvens in treating purpura (ITP). Famous doctor Deng treated purpura (ITP) with combination of Selaginella Involvens and Radix Rumicis, which could strengthenly stop bleeding. While teacher Hu treated purpura (ITP) with combination of Selaginella Involvens and Cynomorium Coccineum, which could reinforce Yang from Yin.2.2Analysis of main prescriptionsAnalysis of main prescription used by two teachers showed that Famous doctor Deng liked to use DangGui BuXue Decoction for tonifing Qi and bloob, while Chief physician Hu liked to use SiJunZi Decoction for strengthening spleen and replenishing qi, which indicated that Reinforcing Qi was the basic treatment of purpura (ITP) in TCM. Famous doctor Deng treated purpura (ITP) with combination of ErZhi Pill with DangGui BuXue Decoction, which focused on supplementing qi and nourishing yin and blood. Chief physician Hu treated purpura (ITP) with combination of Golden Cabinet’s Kidney Qi Pill wand SiJunZi Decoction, which laid special emphasis on Warming Yang and Activating Qi.3. Analysis of development of academic thoughts between famous doctor Deng and my advisor Hu in treating purpura (ITP)3.1Inheriting the academic thoughts of famous doctor Deng Chengshan laid the foundationChief physician Hu inherited the academic thoughts of famous doctor Deng such as the differentiation combined with disease, the pathogenic factors of purpura (ITP) including deficiency and blood stasis, and the association of disease with kidney.3.2Expand ideas by reviewing the ancient literatures of TCM Chief physician Hu attached a great importance to the ancient literatures of TCM, e.g. the source of the blood lie in kidney, strengthening Yang is more important than nourishing yin, Yin can not be activated without Yang.3.3Experience and understanding in the clinical Clinical practice showed that good effects could be achived by Warming Yang and Activating Qi and reinforcing Yang from Yin on purpura (ITP) treated with Golden Cabinet’s Kidney Qi Pill or with prescription added in yang-warming herbs such as Cynomorium Songaricum.ConclusionChief physician Hu has inherited the academic thoughts of famous doctor Deng such as the differentiation combined with disease, the pathogenic factors of purpura (ITP) including deficiency and blood stasis, and the association of this disease with kidney, in the treatment of purpura (ITP). The academic thoughts inherited from famous doctor Deng have been applied in clinical. The academic thoughts of famous doctor Deng in treating purpura (ITP) have been developed by chief physician Hu, which has laid the academic foundation of chief physician Hu by inheriting the academic thoughts of famous doctor Deng, reviewing the ancient literatures of TCM, and understanding in the clinical. This study might be useful to try and explore the inheritance and development of clinical experience and academic thoughts of famous TCM doctors. |
