Effection Of Zhilong Capsule Combined With Candesartan Therapy On Fibrinolysis System And Urine RBP In Early Stage Of Diabetic Nephropathy Rats

Objective:We take the imbalance of plasminogen activator inhibitor(PAI) and tissue-type plasminogenactivator(tPA) as the breakthrough point, to observe whether the Zhilong capsule,especially itunited candesartan cilexetil,whether effectively improve renal function and delay kidneypathological damage.In order to provide a theoretical basis for clinical drug application,weexplore the possible mechanism of diabetic nephropathy (DN), of which the Zhilong capsuleprevent and treat early DN.Methods:We choose SPF Wistar male rats, which were fed with high sugar and high fat diet for4weeks， as our experimental subjects. In addition to the normal control group, the other ratswere given streptozotocin (STZ) intraperitoneal injection of45mg\/kg (IP) in the earlypreparation for model DN rats. The model rats were randomly divided into4groups：modelcontrol group, Zhilong capsule treatment group, candesartan cilexetil treatment group andunited group.In addition having a normal group. After4-week corresponding treatment, ratswere observed through microscope, biochemical test, ELISA to conclude its generalconditions. The following checks were taken:①Fasting plasma-glucose (FPG)；②Serumcreatinine (Scr)、Blood urea nitrogen(BUN)；③Total cholesterol (TC), triglycerides (TG);④Plasma PAI, tPA;⑤Urine micro-albumin (uAlb) and urinary retinol binding protein(RBP)；⑥Body weight(BW)、kidney weight(KW) and hypertrophy of kidney(KI);⑦Kidney tissue pathological changes.All experimental datas were processed with IBMSPSS19.0software.Results:Compared with the normal control group, rats in DN group were increased significantly(p<0.01, p<0.05) in the aspect of FPG、Scr、BUN、TG、TC、PAI、P/t、KW、KI、uAlb andurinary albumin; however, the plasma tissue-type plasminogen activator decreasedsignificantly (p<0.01). Among those groups, the united group has the most significant effect.According to light microscopy results, each treatment group can, to some extent, reduce renalhypertrophy, inhibit mesangial proliferation, and relieve renal damage, especially the united group.Conclusion:Zhilong capsules play an important role in balancing plasma plasminogen activatorinhibitor(PAI) and in reducing TG、TC、BUN、Scr、urinary albumin RBP、uAlb and KI. Itcan relieve the pathological injury in renal tissue of DN rats to protect its kidney. Combinedwith candesartan cilexetil, it will have significant effect on control of early DN.
