Objective:Observation and analysis the clinical efficacy of using traditional Chinese medicine in phases treatment to Chronic severe hepatitis BMethods:Select34cases of chronic severe hepatitis B patients that met the inclusion criteria((early period and intermediate period), and devided them randomly into two groups,16cases in the control group(fall out two cases)and18cases in the treatment group. The control group treated only Western medicine. For the treatment group, additionally, treated with traditional Chinese medicine. We use traditional Chinese medicine in phases for the treatment group. At the beginning of the illness the focus of treatment is eliminating pathogenic factors,and at the late stage the emphases is strengthening the body resistance to eliminate pathogenic factors. Chinese medicines are used by3ways——oral, enema and intravenous infusion. In the first two weeks of the disease, use the Chinese medical to clear heat-toxin, detoxification and cool blood circulation for removing blood stasis. The oral prescription used Ganduqing granule as the basis, and the intravenous infusion of prescription combined Danshen injections and Qingkailing injections, and the enema prescription was Large Chengqi Decoction. Two weeks later, we foucsed on deficient_syndrome. Two weeks later, the treatment group is treated with traditional Chinese medicine to strengthen the body resistance. We lay emphasis on warming yang for dispelling cold to strengthen the body resistance.①For the yin deficiency of the liver and kidney, the oral prescription combines Ganduqing granule and Zeng Ye Tang, the enema prescription used Zeng Ye Chengqi Decoction instead, the intravenous infusion of prescription combined Danshen injections and Shengmai injections.②For the yang deficiency, the oral prescription used Yin Chen Zhu Fu Tang adding with Cinnamon、Rhizoma Zingiberis、 Astragalus、Atractylodes and so on, the enema prescription used Wen Pi Tang instead, the intravenous infusion of prescription used Shenfu Injection.③For the middle type-they are not yin deficiency or yang deficiency, the oral prescription used Ganduqing granule adding with Cassia twig、Fennel、Atractylodes、Herba Epimediiand、Morinda so on, the enema prescription used Xiao Chengqi Tang adding with little Monkshood、Rhizoma Zingiberis and Angelica, the intravenous infusion of prescription used Shengmai injections. The course of treatment is4weeks. After the end of treatment, observe and compare the the degree improvement of biochemical indicator of two group(such as TBIL、PTA、 ALB and so on), and analyze the therapeutic effective rate and symptoms improvement effective rate of two goups。Results:①After four weeks, in the treatment group there were7cases that had a significant effect,11cases had the effect,4cases had no effect. The survival rate and total effective rate of the treatment group was95.5%(21/22) and81.8%(18/22).On the other side,in the control group, the number of the marked improving cases, effective cases,invalid cases was4cases,7cases,9cases. The total effective rate and survival rate was55.0%(11/20)and95.0%(19/20).The total effective rate of two groups did not have a significant difference (P>0.05).②When the treatment course is over,the degree improvement of TBIL and PTA of treatment was better than that of control group,which had a significant difference (P<0.05).The degree improvement of ALB,ALT,AST between the treatment group and the control group did not have a significant difference (P>0.05).③After the end of the treatment, the symptom improvement rate of treatment group such as fatigue,anorexia,loose stool or Constipation was higher than that of control group, and the difference was statistically significant (P<0.05). But symptom improvement rate of abdominal distension, bitter taste, nausea and vomitting did not have a significant difference (P>0.05).Conclusion:Using traditional Chinese medicine in phases to treat chronic severe hepatitis B is an effective method. When we treat the patient, we should pay attention to the deficient syndrome, especially the deficiency of yang. Don not worry the more Chinese medicine used, the worse the situation becomes. |
