ObjectivesDue to traumatic brain injury, stroke and serious craniocerebral diseaseresulting in persistent vegetative state (Persistent vegetative state, PVS) patients isincreasing year by year. Course of disease in patients with PVS long, difficult,expensive care, poor prognosis, treatment is always a difficult problem faced by bothat home and abroad, the need to further improve the success rate of PVS in patientswith arousal, more in-depth research of PVS occurrence, development ofpathological, physiological mechanism, seek more effective method of treatment,shorten the coma patients with stage. This experiment using the experimental cattemporal parietal epidural placement water sac cerebral herniation caused byprinciple, establish PVS animal model, experimental observation of vital signs, catnerve reflex and behavioral changes; use of deep brain stimulation technology (deepbrain stimulation, DBS) stimulated PVS cat thalamus nucleusPARAFASCICULARIS in Hangzhou complex (center median-para fascicularcomplex, CM-pf), and to observe the different experimental cat in blood andcerebrospinal fluid changes of monoamine neurotransmitter dynamic process andvital signs, neural reflex and behavioral changes of DBS in feature, on monoamineneurotransmitters influence, to maintain awareness of the role, providing atheoretical basis for further study of DBS in persistent vegetative the state inpromoting revival effect mechanism.Methods150adult mongrel cats, of either gender, weighing2.0kg±0.5kg. Wererandomly divided into three groups: blank control group with10rats,10rats in thesham operation group,130rats in group PVS. The blank control group, no anytreatment, sham operation group only craniotomy, does not damage the brain tissue, PVS group using temporal parietal epidural placement water sac method to establishPVS animal model, after the success of modeling, experimental observation of vitalsigns, cat nerve reflex and behavioral characteristics; random PVS cat model wasbuilt according to the DBS treatment of different random divided into four groups:treatment group, sham DBS group, high frequency stimulation group, low frequencystimulation group. Treatment of control group were fed normally, without anytreatment, sham DBS group using stereotactic technique in PVS cat CM-pfimplanted electrodes, no stimulation, high frequency stimulation group usingstereotactic technique in PVS cat CM-pf implanted electrodes,60Hz high frequencystimulation, low frequency stimulation group using a stereo set to the technology inthe PVS cat CM-pf implanted electrodes,20Hz low frequency electric stimulation.Using high performance liquid chromatography with electrochemical detectionmethod respectively in each subgroup in1D,5D,10d,15d,20d,25d,30d phase inblood and cerebrospinal fluid (DA) dopamine content change. Statisticalexperimental cats and observed the measuring indexes.ResultsEstablishment of PVS animal model successfully. The cat, changecharacteristics of experimental cats recorded the vital signs, neural reflex andbehavior.2electrochemical detection method by high performance liquidchromatography to detect the content of each group in different time phase ofcerebrospinal fluid, blood DA. The cat was recorded in vital signs, neural reflex andbehavioral changes.3low frequency stimulation group compared with the blankgroup, dopamine content in blood and cerebrospinal fluid was significantlydecreased, the difference was statistically significant (P <0.01). In January4afterDBS treatment, DBS group recovery rate is higher than the group, the difference wasstatistically significant (P <0.05).5low frequency stimulation group, the content ofDA in blood and cerebrospinal fluid from15days significantly increased comparedwith other groups, the difference was statistically significant (P <0.01).Conclusion1using temporal parietal epidural placement water sac caused by cerebralhernia method, successfully established the animal model of persistent vegetativestate. Dopamine content in2the detection of PVS in cerebrospinal fluid of cat waslower than that in normal cats, demonstrated by the dopamine content in CSF andpersistent vegetative state, development effect.3deep brain stimulation technique of bilateral stimulation of the para fascicular thalamic nucleus Centromedianuscompound can obviously increase the content of dopamine in persistent vegetativestate cat in the cerebrospinal fluid, which may be one of the reasons for promotingrevival effect.4according to DBS30days after treatment, low-frequency stimulation group recovery rate is higher than that of each subgroup, confirmed that DBStechnology has the promoting revival effect on PVS cat. Deep brain stimulationtechnology in the future can become one of the methods of vegetative awaking, is ofcertain clinical value, but the operation indications, stimulus intensity, target, thechoice of time need to be further studied.
