Background:Gaomo is a form of massage combined with drug. It is a way of external therapy of traditional Chinese medicine which is based on drug compounding ointments for media effect of preoperative in surface coated with ointment and then is subjected to. Notopterygium cream, developed by Director Xue Mingxin of Department Tuina,produced by our agents section of Jiangsu Province Hospital of TCM, is a new preparation which main component is volatile oil in rhizoma et radix notopterygii that has anti-inflammatory and analgesic clinical pharmacological effects.We observe an effect of cytokines in Gaomo with notopterygium cream treatment on papain-induced inflammatory synovitis of rabbit knee joint to investigate its mechanism.Objective:The aim was to investigate the mechanism of Gaomo with notopterygium cream in treating patients with osteoarthritis of knee joint synovitis by observing notopterygium cream massage with ointment in treating papain-induced inflammatory synovitis of knee joint in rabbits induced by leukotrienes (LTB4), the c-terminal end of type Ⅱ collagen peptide (CTX-Ⅱ) and Interleukin-1(IL-1) impact.Methods:Selected40health New Zealand White rabbits, randomly divided into empty groups, model groups, notopterygium cream dressing groups, tuina groups and gaomo groups, each group of8, model groups, notopterygium cream dressing groups, tuina groups and gaomo groups buide the left knee osteoarthritis model by using papain intra-articular injection,The blank group and the model group were not treated.At modeling for2weeks the notopterygium cream dressing group, tuina group and gaomo group respectively began using notopterygium cream dressing, massage and the gaomo with notopterygium cream treatment. Lequesne MG of knee joint of the experimental animals were respectively assessed before and after treatment.On11th day all animals were executed, the infrapatellar fat pad at the synovial0.5g cut and shear was uniformly centrifuged, supernatant liquor was taken,LTB4and CTX-II were measured by ELISA law,degradation and destruction of left knee synovial tissue and cell of all groups of experimental animals were observed by pathological observation and immunohistochemical semi-quantitative analysis of IL-1Results:Before treatment, blank group was of significant difference(p<0.05) in Lequesne MG knee assessment compared with the other four groups, after treatment tuina group showed significant improvement (p<0.05)in gait, joint activities and joint swelling local reactions, notopterygium cream dressing group showed positive effect in the local response and gait (P<0.05), the gaomo group showed significant improvement (P<0.05) in gait and swelling local reactions; the notopterygium cream dressing group compared with the model groups showed significant difference (P<0.05) in LTB4; tuina group compared with the model groups showed significant difference (P<0.05) in contents of CTX-Ⅱ; compared with the model groups, tuina group, notopterygium cream dressing group and gaomo group respectively showed no difference (P>0.05)in Pathological and immunohistochemical semi-quantitative analysis of IL-1.Conclusion:Ointment masssge therapy method of this study is combined with tuina and notopterygium cream,which is also the mechanism of the rabbit knee arthitis synovitis from the result.In other words, it is relieving inflammatory synovium of knee joint inflammation by reducing inflammation to alleviate the symptoms of osteoarthritis of the knee. However, because notopterygium cream and manipulations are obviously antagonistic, as well as there are allergic reactions occurring in experiment, tuina with cream in the treatment plays a certain role, which is not what we are expected.
