Objective:To evaluate different clinical effects of three inflow occlusion methods in hepatectomy including pringle maneuver(group I), the liver mattress suture (group II) and Glissonean pedicle exclusion (group Ⅲ).Methods:The clinical data of the patients underwent the liver resection with the above liver inflow occlusion methods were retrospectively analyzed. The surgery time, inflow occlusion time, tumor character, amount of intraoperative blood loss, blood transfusion, hepatic function and complications rate were compared for each group. Results:The results showed that the three groups of patients with preoperative, operative time, blood flow occlusion time, tumor characteristics, postoperative blood count and liver function, length of stay and ICU time was no significant difference (p>0.05)Ⅲ patients with blood loss and transfusion were significantly less than Ⅰ, Ⅱ two groups (p<0.05); Ⅱ, Ⅲ group postoperative time AST and ALT improved significantly compared to group Ⅰ (p<0.05), the first3,5days TBIL than I group improved significantly (p<0.05), Ⅱ, and Ⅲ group, complications occurred less than group Ⅰ (p<0.05).Conclusions:In the similar surgery time and inflow occlusion time with other two methods, Glissonean pedicle exclusion method can control the intraoperative blood loss and blood transfusion better, and can promote the patient recovery. Besides, the inflow occlusion methods should be selected based on the practical condition of patients. Ten tables, Thirty-four references.
