Objective:To investigate the alteration of plasma levels of omentin-1、visfatin with the presence of heart failure in old patients with CHD and its relationships with the related factors of CHD, and to explore its clinical significance.Methods:Plasma omentin-1、visfatin levels were measured in90subjects (29SAP cases、30UAP cases and31age-and sex-matched healthy controls, age≥60years) using enzyme linked immunosorbentassay (ELISA) methods..According to the NYHA classification,59CHDs were divided into three groups:functional Ⅰ class11cases, functional Ⅱ/Ⅲ class36cases, functional IV class12cases.Results:(1)The plasma level of omentin-1in coronary heart disease patients was significantly lower than the control group(respectively717.63±229.11ng/l、1115.49±361.41ng/l,P=0.000), and SAP group’s,UAP group’s were all significantly lower than control group(respectively780.32±273.19ng/l、652.81±152.94ng/l,P<0.05),while there was no statistically significance between UAP group’s and SAP group’s (P=0.081>0.05).(2)The plasma level of visfatin in CHD patients was significantly higher than the control group(respectively19.22±6.73μg/l、13.31±1.69μg/l,P=0.002),and SAP group’s,UAP group’s were all significantly higher than control group(respectively17.92±4.19μg/l、 20.43±8.31μg/1P<0.05),while there was no statistically significance between UAP group’s and SAP group’s(P=0.080>0.05).(3) With the increase of NYHA class, the plasma omentin-1level decreased, while Visfatin levels increased gradually,and amongs the three groups,there were significant differences,and the differences were statistically significance (Omentin-1respectively1033.87±277.84ng/l,676.58±137.25ng/l,540.35±59.49ng/l, Visfatin respectively13.00±1.19μg/l、19.08±2.97μg/l、5.23±11.30μg/l,P<0.05).(4) The plasma omentin-1level was negatively correlated with SBP(r=-0.264, P<0.05), positively correlated with HDL-c level(r=0.271, P<0.05); the plasma visfatin level was positively correlated with TC(r=0.292, P<0.05), negatively correlated with HDL-c level(r=-0.266, P<0.05).(5) There was negatively correlation between plasma omentin-1and visfatin levels (r=-0.280, P<0.05); multiple linear stepwise regression analysis showed that omentin-1, visfatin levels all might be affected by HDL-c level; Logistic regression analysis showed that visfatin, CRP, HDL-c may be independent risk factors of CHD.Conclusions:(1) Decreased levels of Omentin-1,and increased levels of Visfatin all may be involved in the occurrence and development of CHD.(2) Omentin-1and visfatin separately may be protection and proinflammatory cytokines, and all may be related to lipid metabolism. (3) Visfatin may be an independent risk factor for coronary heart disease.
