Objective:Pituitary adenomas are the most common type of pituitary disorder.They are are benign neoplasms.Surgical removal of the tumor is the preferred treatment for patients with pituitary adenomas except for that patients with prolactinoma.However,the surgery procedure is complicated.Many complications can happen during the operation or after. Intracranial infection is one of the most severe postoperative complications.This sthdy is performed to explore the risk factors that influence the postoperative intracranial infection of patients with pituitary adenoma.Methods:A retrospective review was performed to analyze the clinical data of459patients who had been surgically managed in our department from January2012to January2014for pituitary adenoma, including the patients’gender, age, diabetes, hypertension,surgical approach(transsphenoidal,craniotomy),operative time,CSF leakage during surgery, defect of the sella diaphragm, amount of bleeding, postoperative drainage pipe, postoperative CSF leakage and postoperative corticosteroids.Data were analyzed using SPSS18.0software.Univariate and multiple logistic regression analysis were performed to determine factors relevant to postoperative intracranial infection of patients with pituitary adenoma.Then estimate the relationship between the independent risk factors and intracranial infection according to the calculated values of Exp(B).Results: Out of the459cases, ICI was occurred in22cases and the rate of ICI was4.8%.Postoperative drainage pipe, postoperative CSF leakage, postoperative corticosteroids and preventive use of antibiotics were factors of statistically significant difference by Univariate Analysis(P<0.05). Logistic regression analysis results:postoperative CSF leakage of this example, is the strongest risk factor[Exp(B)=8.096]. postoperative drainage pipe[Exp(B)=4.785], postoperative corticosteroids[Exp(B)=2.746]cause of postoperative intracranial infection.Preventive use of antibiotics is the protective factor that prevents intracranial infection after Surgical removal of Pituitary adenomas.Conclusions:1.The formation of postoperative intracranial infection of patients with pituitary adenoma are the results of multiple factors working together, and Postoperative drainage pipe, postoperative CSF leakage and postoperative corticosteroids are independent risk factors for postoperative intracranial infection of patients with pituitary adenoma.2. It should be given more attention and prophylactic treatments for the patients with pituitary adenoma with infective risk factors mentioned above.3. Preventive use of antibiotics is the protective factor that prevents intracranial infection after Surgical removal of Pituitary adenomas.
