Object The children with EV71viral encephalitis were divided into severe group and severe critical type group, and healthy children as the control group. The content of peripheral immunoglobulin and lymphocyte subpopulations of three groups of children were detected, and compared the difference between the groups. Then the changes of children’s humoral and cellular immune function with EV71viral encephalitis were analyzed, and the relationship was further explored between immune function and the pathogenesis of severe cases infected EV71virus and disease progression. It is conductive to guide clinical disease prediction and prognostic evaluation, and provides laboratory basis for clinical treatment.Method The children’s emergency room in our hospital received62children with EV71viral encephalitis from2011to2013. They were divided into severe group (simplex virus encephalitis),32cases, and severe critical type group (pulmonary hemorrhage, pulmonary edema, heart and lung failure, etc.),30cases, and selected healthy children in the same age randomly,28cases, as the control group. The content of peripheral immunoglobulin IgA, IgG and IgM were detected by the method of a one-way immunodiffusion, and flow cytometry was used to detect lymphocyte subpopulations-CD3T lymphocytes (%), CD4T lymphocytes (%), CD8T lymphocytes (%), and the ratio of CD4and CD8. Through statistical analysis, different measured values of three groups were researched, and the changes of immune function of children with viral encephalitis EV71was observed.Results The results of humoral immune function detection indicate that the contents of serum IgG, IgA of severe group and critical type group infected EV71virus are significantly lower than the control group (P<0.05), and the difference is statistically significant. The contents of serum IgG, IgA between the children infected severe and critical type EV71virus (P>0.05), and the content of IgM of three groups obviously change, and there is no statistically meaning.The results of cellular immune function detection indicate that the percentages of CD3, CD4and CD8of severe group infected EV71virus increase, and there are statistically meaning between the differences. The percentages of CD3and CD4of critical type group infected EV71virus decrease, the ratio of CD4and CD8decreases than the other two groups, and the differences are statistically significant. Compared with the control group, the percentage of CD8of severe group and critical type group increases, but the changes of CD8are not very noticeable between severe group and critical type group(P>0.05), and there are no statistically meaning between the differences.Conclusion The cellular and humoral immune function of the children with EV71viral encephalitis are abnormal. After EV71infection, the body’s immune response is closely related to neurological complications. The humoral immune function of the children with EV71viral encephalitis reduced, the immune function of severe children enhanced, and critical type children’s immune function reduced. Therefore, it shows that it is gradually suppressed and appears a serious disorder with the condition deteriorated. In addition, the contents of serum IgG, IgA and IgM between the children infected severe and critical type EV71virus don’t change significantly. It speculates that humoral immune function doesn’t play a major role and involved in response of the process, in the transition from severe to critical type, but the cellular immune function is dominant. The dynamic observation of the change in lymphocyte subsets in the clinical, especially, the change of the ratio of CD4and CD8, is conducive to predict the disease and assess the prognosis. Furthermore, the cellular immune enhancer can be used to treat the severe EV71infection disease at the appropriate time. Whether it will be beneficial to prevent disease progression and promote the recovery, is worthy of further clinical validation.
