In recent years, the level of urbanization in Shanghai improves continuously, theeconomy and society develops rapidly and the social security system becomes more andmore perfect. In July,2011, Shanghai canceled the comprehensive social insurance forforeign workers and the social insurance for practitioners in suburban employers, and madethe foreign workers and practitioners in suburban employers into the basic medicalinsurance for urban workers, thus perfecting the medical security system in Shanghai.However, the current basic medical insurance system of Shanghai operates multiplydivided, and there exists census register standard and employment standard which restrictsthe overall development of the urban and rural economy. It also restricts the freemovement of the Labor. So it is important to design the reasonable coordination path assoon as possible to reduce the urban-rural gap and the gap between different groups thateveryone will enjoy the basic health care fairly.This paper firstly analyzes the theoretical basis of the overall development of theurban and rural basic medical insurance system by using the theory of balancing urban andrural development, the theory of institutional change and the theory of public products. Inaddition, makes a comparative study of the three successful modes about the coordinationof the urban and rural basic medical insurance system. Secondly, makes a comparativeanalysis of the current situation of the urban and rural basic medical insurance system inShanghai from five aspects, including the number of people who has the medical insurance,the funding level, the treatment level, the overall level, and the administration of theinsurance. The paper also makes an empirical study about the urban and rural basicmedical insurance system in Shanghai. In addition, the paper makes a study about theurban and rural residents’ satisfaction on the system by using the factor analysis,summarizing the problems of the current basic medical insurance system. Thirdly, thispaper designs the path by integrating and connecting the different medical insurancesystem, including the idea of designing, the model and steps of how to realize the overalldevelopment of the system. Fourthly, the paper measures the individual’s ability to pay and the government subsidy to research the feasibility of the path by using curvilinearregression model and ARIMA time series model. At last, puts forward somecountermeasures and suggestions about how to realize the overall development of theurban and rural basic medical security system combining the concept and content of thedesign, to strengthen government leading and increase the financial input, to optimizesystem design and improve the cohesion mechanism, to strengthen the local legislation andsuperintend a mechanism, to promote the reform of the supporting policies.
