Surface-monitoring equipment, system transmission and information acquisition device make up Maoping Mine ventilation monitoring and supervision system. The system runs well since being used. Analyse and study the system’s reliability can get the hidden failure and what kind of failure can happen, which can guide people to take action to avoid accidents and failure. The content and conclusion of this study are as following.1. Establish Maoping Mine ventilation monitoring and supervision system’s reliability block model. Analyse the system’s reliability based on the method of reliability block. It can be contained that the reliability of Maoping Mine ventilation monitoring and supervision system is 1.2. Analyse the failure mode, effects and criticality of Maoping Mine ventilation monitoring and supervision system from three aspects, surface-monitoring equipment, system transmission and information acquisition device by failure mode, effects and criticality analysis (FMECA).3. Take Maoping Mine ventilation monitoring and supervision system failure as top event. Build the system’s fault tree based on failure mode and effects and analysis (FMEA). Then, use fault tree analysis (FTA) to do qualitative analysis and quantitative analysis of the system’s fault tree.4. Qualitative analysis of fault tree indicates the system has 78 kinds of events combinations which can cause system failure and 2 ways to avoid the failure. By the fault tree’s qualitative analysis, the probility of system failure is P(T)= 0.001747, basic events X40 and X41 have the maximum probability importance and critical importance among all the basic events, which are Ig(40)=Ig(41)= 0.08690581, Igc(40)=Igc(41)= 0.497487437.5. It can take effective measurements to maintain the system well when a failure or a hidden failure occures. Besides, reducing the higher failure rate event such as the CPU of monitoring host cooling bad can also be an effective way in reducing the incidence of system failure. Therefore, it can propose countermeasures from three aspects of people-equipment-management to avoid Maoping Mine ventilation monitoring and supervision system failure and guarantee the system’s normal operation.
