Research On Japanese Nursing Care Insurance

When the elderly lose the ability of living independently, what they need most is the appropriate and considerate Long — Term — Care to maintain their esteem.In the ancient society, elderly citizens’life can rely on their family’s care. While in the modern society, along with the development of industrialization, family cannot shoulder the responsibility of Long — Term — Care the elderly all by themselves, which consequently influenced the economic development and social stability. To solve this problem, Germany, as the first country to do so, has established the long-term care system for the elderly. Following Germany, Japan also established the system of Long—Term—Care with its own distinctive national features. Afterwards, Korea and Taiwan also established the similar systems for caring the elderly. Insurance of Long — Term — Care has been accepted by plenty of governments serving as the "sixth insurance of social security". As a developing country, China is confronted with a very serious aging society. The phenomenon of getting old before becoming wealthy is forcing us to take actions as soon as possible to meet the enormous needs of Long—Term—Care for the elderly. One option of tackling the problem of the elderly Long—Term—Care is to establish the system of Long—Term — Care insurance. Japan has similar features on the aging speed and population structure as us; in addition, we both belong to the Confucius Cultural circle in east Asia. Therefore, their experience has significant reference for us.From the view point of welfare pluralism, this thesis conducts a detailed analysis on the two parts of Japanese Nursing-care System:the welfare thoughts and the system content. In the part of studying the welfare thoughts, this thesis divides the Long—Term—Care for the Japanese elderly into three phases:the family Long—Term—Care phase, national Long — Term—Care phase and the Long—Term—Care insurance according to the development order of Japanese welfare thoughts. In addition, this thesis comes up with corresponding welfare thoughts with the three phases. In the part of studying the system content, this thesis makes a detailed a research on the background, content, purpose of the establishment of Long—Cerm — Care insurance system and the cooperative relationship among the plural subjects in the Long — Term — Care insurance from the view point of the welfare pluralism-advocated "socialization", "marketization" and "decentralization", "participation".This thesis concludes that, the Long — Term — Care insurance system of Japan is implemented to ease the financial burden of central government, reconstruct the social security system and achieve the socialization of the elderly Long — Term — Care under the changing circumstances of its social, political and economic situation. This system achieved a favorable performance after being established. But still, it also brought the problems of "voluntary failure", fair dilemma, deterioration of population structure."Stones form other hills may serve to polish the jade of this one." We can learn lessons from the aspects of system idea, operation mode, Long — Term — Care talents education of Japan to establish the care system for the elderly Chinese.
