Objective:The purpose of this study was⑴to survey the emotional state and sleep quality of AMIpatients after PCI, and which factors that may influence patients’ sleep quality;⑵to studywhether sleep quality could be influenced by positive effect or (and) negative effect, andon which aspects of affect;⑶to find out the effects of Music on acute myocardialinfarction patients after PCI.Methods:Convenience sampling method was employed. One hundred and five AMI patients afterPCI who came from cardiovascular care unit of a tertiary comprehensive hospital inGuangzhou during January to April in2011. The volunteers enrolled in this study weremostly men (88.57%) and Chinese (100%), age from31to83years old (61.61±11.40),days in CCU from3days to6days (3.67±0.70)."PCI Patients Questionnaire" was usedto survey patients general situation and its sleep quality infect factors,"AIS" was used toinvestigate the scores of patients’ sleep quality and "PANAS" was used to survey patients’positive and negative emotions state.100cases of patients after PCI whose score of AIS isat least4in included in the intervention. According to hospital patients order, each patientcorresponding a number, odd number into music intervention group(A group,50cases),even number into the routine treatment group (B group,50cases), Group A playing musicduring12:30-13:00and22:00-22:30,30minutes every time, twice a day, volume<60decibels, the advisable volume was adjusted as which patients feel comfortable; GroupB provided conventional nursing in CCU. The" AIS" was used to survey patients sleepquality score; The "PANAS" was used to survey the condition of positive and negativeemotions; and the signs of life such as blood pressure, heart rate and breathing patientswere record before and after the intervention. SPSS13.0was employed to carry on the statistical description and statistical analysis, and the factors Logistic regression was usedto analysis the data.Results:⑴Patients with AMI before and after PCI there all exists different degree of sleepdisorders, there is no significant difference before and after PCI (P>0.05). The factorswhich affect the patient sleep quality including: environmental factors, physiologicalfactors and medicine factors. And each factors have significant difference before and afterPCI (ρ <0.05).⑵Comparing with good sleep patients, Patients who have different degree of sleepproblems experience high negative emotions, and low positive emotions. The score ofAMI patients’ sleep quality have positive correlation with negative emotions (r=0.34, P<0.01), and negative correlation with positive emotions (r=-0.31, P <0.01). Theemotion of interested and enthusiastic could be considered as prospective factors of thepatients sleep quality, However the irritability emotion could induce patients’ sleepdisorders.⑶After intervention, The results shows that: the participants in A group, whose negativeemotion, sleep quality, blood pressure, heart rate and breath rate all decreased more thanthe B group’s (P <0.05), and also the positive emotion got higher score than B group(P <0.05). In A group, excepting alert emotion, all the other positive emotions improvedhigher than B group (P <0.01). Two group patients have no significant differencedecline in guilty, hostile and ashamed(P>0.05). However A group patients’ othernegative emotion scores all lower than B group(P <0.05).Conclusions:⑴AMI patients before and after PCI exists different degree of sleep disorder, there wereno significant difference between two groups; Influencing the patients sleep quality factorsincluding: environment, physiology, medicine and emotional factors.⑵Comparing with good sleep patients, Insomnia or insomnia patients experience therelatively high negative emotions and low positive moods. Positive emotions could promote sleep, negative emotions might inhibit sleep, We should focus on the impact ofpositive and negative emotional state for AMI patients’ sleep quality. Thus we couldprovide effective care for AMI patients with psychological intervention, promote thepatients’ sleep quality, and promote patients’ physical and mental health.⑶Music intervention could significant promote positive emotions such as interested,excited, strong, enthusiastic. Induce negative emotions such as distressed, upset, guiltyand so on.⑷Music intervention could significant release the negative emotions, improve positiveemotions, promote sleep quality, lower blood pressure, heart rate and breath rate of AMIpatients after PCI who live in CCU. Music intervention is beneficial to the physical andmental rehabilitation.⑸We should also provide careful psycho-nursing in order to enhance the effect ofmusic intervention.
