ObjectiveTo explore the role and significance of Fas, FasL and Bcl-2protein in theprocess of immune escape of hepatoma cells,by detecting their protein expressionand apoptotic index in primary HCC and its adjacent tissues combination withclinical pathology parameters.MethodsImmunohistochemical technique was used to detect protein expressionproportion of Fas、FasL、Bcl-2in30cases of primary HCC and its adjacent tissuesand10cases of benign liver group. Apoptotic cells were detected by terminaldeoxynucleotidyl transferase-mediated dUTP nick end labeling(TUNEL). Theexperimental data with clinicopathologic parameters was analyzed statistically.Results1. The positive rate of FasL protein expression in HCC group and its adjacenttissues group and benign liver group was66.67%(20/30)、40.00%(12/30)、20.00%(2/10). The differences between groups were statisticallysignificant(P<0.05).The expression of FasL protein in HCC was no significantcorrelation with the patient’s age, AFP level,HBsAg, tumor thrombus,pathological grade and tumor size (P>0.05).2. The positive rate of Fas protein expression in HCC group and its adjacenttissues group and benign liver group was43.33%(13/30)、70.00%(21/30)、10.00%(1/10). The differences between groups were statisticallysignificant(P<0.05).The expression of Fas protein in HCC was no significantcorrelation with the patient’s age, AFP level,HBsAg, tumor thrombus,pathological grade and tumor size (P>0.05). 3. The positive rate of Bcl-2protein expression in HCC group and its adjacenttissues group and benign liver group was36.67%(10/30),73.33%(22/30),10.00%(1/10). The differences between groups were statistically significant(P<0.05).The expression of Bcl-2protein in HCC was no significant correlationwith the patient’s age, AFP level,HBsAg, tumor thrombus, pathological gradeand tumor size (P>0.05).4. The average apoptotic index in30cases of HCC and its adjacent tissues was1.73±0.63%and6.14±1.44%, the difference was significant (P <0.05); thedifference between Fas, Bcl-2of negative and positive expression in HCC andthe death index was no significant.5. Expression of Fas and Bcl-2protein in HCC tissues was negativelycorrelated(r=-0.366,P<0.05).Conclusions1. HCC tissues escape immune surveillance system to reduce the formation ofapoptotic cells by low expression of Fas weaken the role of cytotoxic T cellsand high expression of FasL removed part of the cytotoxic T cells, promotingthe development of HCC.2. Adjacent tissues of HCC delay the process of transformation to HCC by way ofthe high expression of Fas to promote apoptosis; The abnormally highexpression of FasL may be derived from tumor cells, in order to promote themalignant transformation of the adjacent tissues.3. Expression of Fas and Bcl-2protein in HCC tissues was negativelycorrelated(r=-0.366,P<0.05), but Bcl-2protein may not play a important role ofthe formation of HCC.4. No significant correlation of Fas, FasL, Bcl-2expression and its clinical andpathological features in HCC tissues. 5. Expression of Fas, FasL and Bcl-2protein in HCC and its adjacent tissues aresignificantly higher than the benign control group, indicating that the expressionof the anomalies and imbalances is closely related to progress of HCC,but alsosuggesting that the progress of HCC is multifactorial pathways for pathogenesis.
