| Objective:To systemically assess the feasibility and safety of the complete mesocolicexcision for patients with Colon cancer.Methods:A computer-based online research of prospective, randomized or nonnonrandomized, controlled studies addressing complete mesocolic excision versustraditional surgery published the last five years was performed in electronicdatabases（Wan fang Database, China National Knowledge Infrastructure,ChineseMedical Current Contents,VIP, Pub Med, Medline, Ovid, Elsevier, ISI Web ofKnowledge, Cochrane Database of Systematic Reviews）.With strictly screeningaccordance to the standard, assessing the quality of these studies, data were extractedfrom these trials and analyzed by the Review Manager5.1software at last.Results:A total of eight nonrandomized clinical trials, involving a total of1209patients（including615patients in CME group and594patients in control group）, wereidentified. Meta analysis showed that:(1) Intraoperative blood loss in the CMEgroup was less than traditional surgery [WMD=13.05,95%CI (25.03,1.07),P=0.03<0.05], There was no obvious difference in the operation time [WMD=0.46,95%CI (26.50,27.41), P=0.97>0.05];(2) Among those postoperative pathologicspecimens, lymph node retrieved, the average length of large bowel resected, thearea of mesentery resected, the high vascular tie, in the CME group were significantmore than that in the traditional surgery;(3) There were no differences in time tofirst exhaust, hospital stay between the two groups(P=0.87,P=0.05);(4) Thepostoperative complications was not increased in the CME group versus traditionalsurgery［OR=1.07，P＝0.74］.Conclusion:Meta analysis show that complete mesocolon resection is completely safe and effective, in accordance with concept of the embryonic anatomy, oncological surgeryand delicate surgery, is expected to become a standardization operation method forcolon cancer. |
