ObjectiveTo analyze the relationship and between chronic heart failure(CHF) and the heart and kidney syndrome in traditional Chinese Medicine. To explore such relationship, cardiac function and myocardial pathological morphology of CHF rat were evaluated before and after the application of yixinbushen, a method of improving cardiac and kidney function, which could provide experimental basis for the clinical application of yixinbushen.MethodsFrom January1st2012to December31st2013,200cases CHF enrolled into the3rd Affiliated Hospital of Guangzhou University of Chinese Medicine were admitted to our study. The TCM syndrome characteristic of all patients were collected and analyzed by factor analysis. Models CHF male SD rats were established and randomly divided into4groups:Yixin group, Bushen group, Yixinbushen group and positive drug group (Captopril and Betaloc). Normal male SD rats and sham operation male SD rats were enrolled as control groups at the same time. All groups had been treated correspondingly for8weeks. Function and myocardial electron microscope were examined and compared within groups at4th week and8th week respectively.ResultsClinical trial results showed29symptoms may represent the CHF characteristics by factor analysis. Syndromes of heart and kidney were highly expressed. Syndrome of spleen and kidney were highly expressed in early stage (class II), and heart and kidney were highly expressed in late stage (class III and IV). Left ventricular ejection fraction (LVEF) in heart, kidney and heart combined kidney syndromes of TCM had significant differences (P<0.05), especially in heart combined kidney syndromes, LVEF was the lowest (P<0.05). Animal test showed:In the early stage (about4weeks after CHF model were established), the heart rate had no significant differences between CHF rat groups (P>0.05). Compared with early stage, the heart rate of control groups, yixinbushen group and positive groups had no significant differences (P>0.05), while that of bushen group and yixin group were decreased (P<0.05). In early stage and late stage, systolic blood pressure(SBP),diastolic blood pressure(DBP) and left ventricular systolic pressure(LVSP)were decreased(P<0.05). In late stage, SBP, DBP and LVSP in Yixinbushen group were lower (P<0.05).Compared with early stage, SBP and DBP were lower in yixinbushen and positive groups in late stage(P<0.05), LVSP in yixin and bushen groups were lower(P<0.05),LVSP in yixinbushen and positive groups had no significant differences(P>0.05).Compared with control groups, left ventricular end-diastolic pressure (LVEDP) in yixin group, bushen group, yixinbushen and positive groups were higher in early stage(P<0.05). Compared with early stage, LVEDP in yixin and bushen groups were higher (P<0.05) and yixinbushen and positive groups did not change (P>0.05) in late stage.Compared with control groups,+dp/dtmax in yixin, bushen and yixinbushen groups were lower (P<0.05). Compared with early stage,+dp/dtmax in yixin and bushen groups were increased (P<0.05), yixinbushen and positive groups did not change (P<0.05). Compared with the control groups, myocardial cells was significantly impaired in yixin, bushen and yixinbushen groups. Compared with yixin and bushen groups, the impaired myocardial cells were obviously slight.ConclusionThe syndromes of chronic heart failure were mainly shown as deficiency of heart or/and kidney. TCM of Yixinbushen (Yixin combined with bushen) is better than that of Yixin or Bushen alone.
