Influences Of Vidian Neurectomy On Peripheral Blood Monocyte Th1、Th2、Th17、Treg Cells In Allergic Rhinitis

Objective:To explore the changes of Th1,Th2,Th17,Treg cells of peripheral blood in patients of allergic rhinitis before and after vadian neurectomy through flow cytometry.Method:Experimental group including20patients with severe persistent allergic rhinitis (postoperative group1and group2) were carried out endoscopic vadian neuroctomy. The operational efficacy was evaluated before and after operation by "Visual analogue scale(VAS)[" and "Rhinoconjunctivitis quality of life questionnaire (RQLQ)". and10healthy people as the control group,other10patients with severe persistent allergic rhinitis as preoperative group. the numbers of Th1,Th2,Th17,Treg cells in peripheral blood of all the people were detected by flow cytometryResult:People in the postoperative group1(6months after operation) were tested by VAS and RQLQ, The total effective rate both was100%. postoperative group2(1year after operation) were tested by VAS and RQLQ, the total effective rate was70%and90%partly.Take group1and group2as a whole,the total effective rate tested by VAS was85%and95%tested by RQLQ.The percentages of CD3+/CD8-/IFN-γ (Th1) and CD3+/CD8-/IL-4(Th2) in peripheral blood were (17.21±3.41),(3.01±0.92) in control group, and were (9.89±1.02),(8.64±2.13) in preoperative group. This percentages were (10.22±1.37) and (8.31±2.89) in postoperative group1, and were (10.03±1.51) and (8.54±1.91) in postoperative group2. There was statistically significant difference between control group and preoperative group、postoperative group1、postoperative group2(P <0.05). There was no-statistically significant difference between before and post operation (P>0.05). The difference was also no-statistically significant difference between postoperative group1and postoperative group2(P>0.05).The percentages of CD3+/CD8-/IL-17and Foxp3+CD4+CD25+Treg in peripheral blood were (0.51.21±0.20),(6.01±0.83) in control group, and were (1.60±0.34),(2.59±0.43) in preoperative group. This percentages were (1.51±0.51) and (2.70±0.36) in postoperative group1, and were (1.51±0.23) and (3.01±0.17) in postoperative group2. There was statistically significant difference between control group and preoperative group、postoperative group1、postoperative group2(P <0.05). There was no-statistically significant difference between before and post operation (P>0.05). The difference was also no-statistically significant difference between postoperative group1and postoperative group2(P>0.05).Conclusion:(1) The content of Th1,Treg cells in peripheral blood in allergic rhinitis patients was decreased significantly when Th2,Th17cells was increased significantly,Thl/Th2and Thl7/Treg cells present imbalances.(2) Endoscopic vadian neurectomy in the management of allergic rhinitis can not change the numbers of Th1,Th2,Th17,Treg cells significantly, the imbalanced in Thl/Th2and Th17/Treg was not improved significantly.
