Objective:To compare the therapeutic outcome of Abdominal Aortic Aneurysm(AAA) by open surgery and endovascular repair(EVAR), and analyze the indications of traditional open surgery and EVAR.Methods:Clinical data of98patients with AAA from February2010to February2013in Tianjin Medical University General Hospital were retrospectively analyzed. Preoperative general situation, intraoperative condition, perioperative complication, hospital expenses and long-term complications were comparatively analyzed to the group of open surgery and EVAR. The clinical results of2groups were evaluated.Results:Diagnosis of AAA to98patients were done by CTA and MRA.30cases were performed open repair (22elective surgery and8emergent) and68patients were repaired by elective EVAR. There were no significant difference in the aspects of gender, average age, aneurysm diameter, smoking history, comorbidities such as hypertension, diabetes, coronary heart disease and COPD between the two groups (P>0.05). The blood loss, hospital stay and perioperative complications of important organs in EVAR group were significantly less than those in open group (P<0.05). The hospital costs and long-term complications in EVAR group were significantly more than those in open group (P<0.05).Conclusion:1. Compared with traditional open surgery, EVAR takes the advantage of less invasion, less blood loss, rapid recovery and less perioperative complications and it is suitable for patients of old age and with poor general condition. The main drawbacks of EVAR are high cost and more long-term complications.2. Open surgery takes the advantage of less cost and long-term complications and it is suitable for the patients of young age, less comorbidities and without the condition of EVAR.
